摘  要:目的:研究糖尿病胰腺在CT形态学上的改变。材料与方法:分析了88例糖尿病和360例正常对照组的胰腺CT及临床资料,对各组胰腺头、体、尾3个部位作了前后径的测量,并对胰腺形态与结构进行了比较,以及统计学上的处理。结果:糖尿病各组胰腺均有不同程度的萎缩,其中以Ⅰ型(IDDM)及Ⅱ型(NIDDM)中胰岛素治疗组及C肽低于正常组的胰腺缩小最为明显,萎缩部位以体部出现早,且较显著。胰腺萎缩与病程的长短有关。结论:内分泌胰(胰岛)对外分泌胰(腺泡组织)有营养作用。因此糖尿病胰腺可有不同程度的萎缩。胰腺的CT检查有助于发现糖尿病高危病例,在NIDDM患者根据胰腺萎缩的程度及部位。Objective: To assess the morphological changes of the diabetic pancreas on CT.Materials and Methods: CT examination with clinical analysis of 88 diabetic patients and 360 control subjects were studied. The A P diameters of the head, body and tail of the pancreas and pancreatic structure (contour and density) in CT image of two groups were compared. The statistical difference between diabetic groups and control groups were estimated.Results: All parts of the pancreas tended to be smaller in diabetic patients. It was more pronounced in type Ⅰ(IDDM) and type Ⅱ (NIDDM) treated with insulin and low serum C peptide group. The degree of reduction in the diameter of various portion of pancreas are varied. The atrophy of the body in the pancreas is more obvious and appeared more earlier. Atrophy of the pancreas is correlated significantly with the duration of the disease.Conclusion: The endocrine pancreas (Islets) has a trophic effect on exocrine pancreas (Acinnar tissue). So the diabetic pancreas might have various degree of atrophy. CT could help to detect those patients who are at risk for diabetes mellitus. According to the degree and position of the atrophy in the pancreas among the NIDDM patients, CT could also provide the indication for insulin treatment.
