摘  要:目的 调查南昌市一个社区人群臂围,并观察袖带臂围匹配与否对袖带法血压测值的影响。方法 由两位经过专门培训的医务人员,用12cm×22cm袖带测量25~90岁社区居民2041名血压、右上臂的臂围、身高、体质量、体质量指数(BMI)。按年龄分为≤35岁组(≤35)、36~45岁组(35~)、46~55岁组(45~)、56~65岁组(55~)、〉65岁组(〉65)。208名冠脉造影患者同步测量主动脉根部(直接)和袖带法肱动脉(间接)血压,计算两者的差值(直-间血压差)。结果 1)该社区成人臂围为17~37cm;2)袖带臂围不匹配(〉26cm)组的收缩压和舒张压测值分别比匹配(≤26cm)组高6mmHg和4mmHg;不匹配组的直-间收缩压差比匹配组小2.7mmHg。结论 1)小号袖带对49.0%的南昌地区人群过小;2)使用小号袖带可能高估臂围〉26cm人群的收缩压。Objective To investigate the arm circumference (AC) of a cohort of adult population in Nanchang area and its influence on blood pressure (BP) measurement. Methods BP was taken at right arm for 3 times in 2041 adults from 25 to 90 year old. The population was classified into 5 age groups. The AC, body height, body weight were measured. In 208 patients the invasive ascending aortic BP and noninvasive brachial BP were simultaneously measured for 3 times during coronary angiography. The difference of invasive and noninvasire BP was calculated as index of BPi-n. Results 1)The range of the AC was 17-37 cm in this population and the small adult cuff (size:12 cm×22 cm) was unsuitable for 49.9% of this population. 2)Using the inappropriate size of cuff in subjects with AC 〉26 cm will overestimate the BP, with SBP 6 mm Hg higher than that group with AC ≤26 cm, and DBP 4 mm Hg higher. 3)The SBPi-n in group with AC 〉26 cm was smaller than that in group with AC ≤26 cm, indicating that SBP was overevaluated for 2.7 mm Hg in larger AC group as comparison with smaller AC group. Conclusion 1)The small adult cuff is unsuitable for 49.9%of population in Nanchang area; 2)The SBP may be overestimated in persons with AC more than 26 cm when a small adult cuff is used.
