SOLIS在颈椎前路融合术中的应用

摘　　要：目的探讨新型颈椎前路融合器SOLIS(PEEKCage)在颈椎前路融合术中的应用效果。方法对18例脊髓型颈椎病、12例颈椎间盘突出症患者采用颈椎前路减压融合术,小切口入路(3～4cm),椎间盘及部分椎体后缘切除后,保留椎体终板,植入带自体松质骨的SOLIS。以手术前后X线片及日本骨科学会(JOA)评分评价疗效。结果30例随访6～18个月,平均12.5个月,置入的Cage位置良好,无移动及脱出迹象,病变椎间隙高度恢复正常,无椎间高度丢失发生,所有节段均于术后3～8个月骨性融合。术前JOA评分平均10.40分,术后平均14.86分,二者差异有统计学意义(P<0.01)。结论颈椎前路融合器SOLIS具有良好的生物相容性,手术创伤小,能有效地恢复颈椎高度,融合率高,融合后稳定性好,神经功能改善优良率高。Objective To evaluate the application of SOLIS in the surgery on anterior intervertebral fusion. Methods The operations and results of the surgery on anterior intervertebral fusion in 18 patients with cervical spondylotic myeolopathy and 12 cases with cervical intervertebral disc herniation were reviewed. SOLIS Cage filled with autocancellous bone were implanted between the cervical vertebrae and the endplate were preserved after the cervical intervertebral disc and some parts of the posterior border of the vertebral body were removed. Every patient had a short neck incision. Recovery of nerve function was observed by JOA score and the fusion state, stability and changes of intervertebra height of fusion segment were evaluated by X ray. Results The movement of the implanted SOLIS Cage were not found in the duration of follow - up ranged form 6 to 18 months with an averange of 12. 5 months. All the involved heights of the cervical intervertebral spaces were restored and loss of height of intervertebral didn＇t happen. Bony fusion could be found 3 - 8 months post - operative. The average JOA was 10. 40 before operation and 14. 86 after operaion. There was highly significant variation between the former and the latter （ P 〈 0. 01 ）. Conclusion SOLIS has an excellent biocompatibility, microtrauma and can restore and maintain the intervertebal disc hight effectively with satisfactory fusion rate. The patients can restore quickly after the application of SOLIS for anterior intervertebral fusion and excellent results show in short term.
