芪参降糖胶囊改善2型糖尿病胰岛素抵抗的临床观察

摘　　要：目的:观察芪参降糖胶囊对2型糖尿病患者胰岛素抵抗的影响。方法:将179例2型糖尿病胰岛素抵抗患者随机分为对照组77例和治疗组102例,两组患者均采用口服二甲双胍治疗,治疗组在此基础上加服中药芪参降糖胶囊。在治疗期间均按饮食疗法的要求,固定每日总热量摄入,并固定运动量。两组患者分别于治疗前后进行空腹血糖、胰岛素、血红蛋白以及血脂检测,并计算胰岛素抵抗指数。结果:两组患者治疗后疗效比较,对照组总有效率为68.83%,治疗组为82.65%,两组患者治疗后疗效比较有显著性差异(P<0.05)。两组患者治疗后FPG、HbA1C、EINS、IAI、TG、TC水平明显下降,与治疗前比较有显著性差异(P<0.05,P<0.01),治疗后组间比较也有显著差异(P<0.05,P<0.01);治疗后HDL-C水平均有所升高,但治疗组治疗后HDL-C水平与治疗前比较有显著性差异(P<0.05),而对照组无显著性差异,组间比较也无显著性差异。结论:芪参降糖胶囊联合西药二甲双胍治疗糖尿病胰岛素抵抗,疗效显著,值得在临床上推广应用。Objective.To observe the effects of Qishenjiangtang capsule on insulin resistance in type 2 diabetic patients. Methods： 179 cases of type 2 diabetic patients with insulin resistance were divided randomly into two groups, 102 in the therapeutic group and 77 in the controlled group. Patients in two groups were given the treatment respectively . Measure the Fasting plasma glucose （FPF）, Fasting insulin （FINS）, HbA1C, Triglyceride（TG）, Total cholesterol （TO）, High-density lipoprotein cholesterol （HDL-C）, and caculuted the Insulin allergic index （IAI）. Results： （1） In the therapeutic group, 37 cases presented significant curative effect ,47 cases got improved and 18 cases were with invalid effect on the decrease of plasma, which in the control group were respectively 21,32 and 24. The total effective rates in the two groups were 82.65 % and 68.83 % respectively. The treatment of the decrease of plasma in both groups was verified by chi-square test（P〈0.05）. （2）The IAI of the therapeutic group was significantly improved（P〈0.05）, and the FINS was obviously decreased （ P 〈 0.01）. There was significant difference between the two groups（P〈0.05）. （3） The TC and TG of the therapeutic were obviously decreased （P〈0.01）. There were significant difference between the two groups（P〈0.05）. Conclusion： It is more effective for improving the dyslipidemia, insulin resistance and increasing the sensitivity of insulin by Qishenjiangtang capsule combination with Metformin than only use of Metformin.
