头孢吡肟治疗中、重度感染患者383例的临床疗效及安全性评价

摘　　要：目的评价头孢吡肟治疗中、重度感染的疗效及安全性。方法选择中、重度急性呼吸系统、泌尿系统及其他感染患者为研究对象,静脉使用头孢吡肟7～14天,每日2.0～4.0g,评价临床疗效、细菌学疗效及安全性。结果共入选383例,完成360例,其中呼吸系统感染175例,泌尿系统感染157例,其他感染28例。研究显示头孢吡肟对中、重度呼吸系统、泌尿系统和其他感染的临床有效率分别为84.57%、94.27%和96.43%;治疗后细菌阴转率94.42%,细菌清除率为95.27%。致病菌株的纸片药敏结果显示,致病菌株对头孢呋辛、头孢他啶及头孢吡肟的敏感率分别为49.09%、75.55%和93.82%。头孢吡肟总的不良反应发生率为9.55%,主要不良反应包括恶心(1.86%)、皮疹(1.33%)、单项或双项转氨酶升高(4.51%)等。结论头孢吡肟对中、重度感染患者具有良好的临床治疗效果,且安全性较好,为治疗急性细菌感染的安全、有效的抗菌药物。Objective To evaluate the efficacy and safety of Cefepime on the patients with moderate or serious infections. Methods Selected patients with moderate or severe acute respiratory system infection,or urinary system infection or other system infection. Intravenous injection Cefepime 2.0-4.0g per day. The therapeutic duration was 7- 14 days. To evaluate the clinical efficacy,bacterial efficacy, and safety of Cefepime. Results A total of 383 patients,which include 175 patients with respiratory system infection, 157 patients with urinary system infection, 28 patients with other infection, were brought into, and 360 of them finished the study. In the therapy of patients with moderate or serious respiratory system infection,urinary system infection and other system infection, there showed the efficient rate was 84. 57%,94.27%, 96.43% respectively. After treatment 94. 42% bacteria became negative, and 95. 27% bacteria were cleared. Slip drug sensitivity test showed that the sensitive rate of the pathogenic strain to Cefuroxime,Ceftazidime,Cefepime were 49.09%, 75. 55%,93. 82% respectively. The total rate of adverse effects was 9.55 %. The main adverse effects include nausea （1.86 %）, rash （1.86 %）, increase of one or two transaminase （4.51%） ,and so on. Conclusions The Cefepime in treating patients with moderate or serious infections was valid and safe. It is a good antibiotics in treating acute bacterial infection.
