876例病人围手术期应用抗菌药物情况分析

摘　　要：目的:分析和评价外科手术病人抗菌药物使用情况。方法:收集2005年1-6月上海市长宁区中心医院876例外科手术出院病历资料,采用回顾性调查方法对资料进行统计分析。结果:Ⅰ类切口手术抗菌药预防用药率100%。Ⅰ类切口手术无指征用药占42.86%(102/238)。存在手术预防用药选择不当(擅自选用广谱抗菌药物,频繁换药,盲目联合用药)、初次给药时间不当和用药持续时间过长等问题。结论:本院外科围手术期应用抗菌药物存在不合理性,亟需根据《抗菌药物临床应用指导原则》进行规范化管理。Objective： To analyze and evaluate the use of antibacterial agents in patients in perioperative period in the department of general surgery. Methods..A retrospective investigation was performed by collecting medical records of 876 cases of surgical operation from Janurary to June 2005 in Central Hospital of Shanghai Changning District. Results：In type I incision operation, the rate of prophylactic use of antibacterial agents was 100% and administration without indication accounted for 42.86~~ （102/238）. Some problems, such as irrational choice of antibacterials （e. g. application of broad-spectrum antibacterials without authorization, frequent change of drugs and blind drug combination）, inappropriate timing of begining of administration and prolonged drug therapy,still existed. Conclusion： Irrational use of perioperative antibacterial agents existed in this hospital. It is necessary to standardize administration of prophylactic antibacterial agents according to ＂Guideline on Clinical Use of Antibacterials＇.
