高血压患者IRS-1基因多态性及C反应蛋白的相关性分析

摘　　要：目的:研究胰岛素受体底物-1(IRS-1)G972R与C反应蛋白(CRP)水平和高血压病的关系。方法:选择原发性高血压患者120例及正常对照者100例,测定身高、体重、收缩压、舒张压、空腹血糖、血浆胆固醇、甘油三酯及CRP。提取DNA,采用链式反应-限制性片段长度多态性(PCR-RFLP)技术测定IRS-1基因G972R多态性。结果:实验组BMI、血糖、甘油三酯、胆固醇、低密度脂蛋白及CRP水平明显高于正常组。两组IRS-1G972R多态GG、GR基因型频率及G、R等位基因频率差异均无统计学意义(P>0.05)。GG、GR基因型间体重指数、血糖、血脂及CRP水平无明显差异。经多因素Logistic回归分析,CRP水平与IRS-1基因型及等位基因频率尚无显著相关性。结论:尚无证据表明IRS-1基因G972R多态性与CRP水平和原发性高血压存在直接关系。Objective To investigate the association of G972R polymorphism of insulin receptor substrate - 1 （ IRS - 1 ） with CRP level and essential hypertension. Methods The study enrolled essential hypertensive （ n = 120） and normotensive （ n = 100） subjects. Body weight index （BMI）, systolic blood pressure （SBP） and diastolic blood pressure （DBP） were recorded. The level of serum glucose, triglyceride, cholesterol and C - reactive protein （CRP） were measured. Genome DNA was extracted from white blood cells. The fragment of IRS - 1 was amplified with pelymerase chain reaction （PCR）. The amplified products were digested with restriction endonuclease. Rusults The BMI and the levels of serum glucose, triglyglyceride, cholesterol and low -density llpeprotein cholesterol in essential hypertensive patients were significantly higher than those in normotensive subjects. There were no association in the frequency of IRS - 1 genotype GC, GR with hypertensive and normotensive subjects （X^2 = 0.576 ,P 〉 0.05 ）. There was not significantly different in the frequencies of G and R between two groups （X^2 = 0.571, P 〉 0.05 ）. Further analyses showed no association between GG and GR genotypes in BMI and the levels of serum glucose, lipids and CRP. The logistic regression showed there was no association between the frequencies of IRS - 1 and CRP. Conclusions No evidence was found to suggest an association of the IRS - 1 G972R pelymorphism with CRP level and essential hypertension.
