摘 要:目的观察冠心病(CHD)患者血浆脂联素(APN)与血管内皮功能指标内皮素-1、一氧化氮(NO)、假血友病因子(vWF)水平变化并分析其相关性。方法分别采用酶联免疫吸附法、放射免疫分析法、硝酸还原酶法测定28例年龄和性别相当的健康体检者和60例CHD患者血浆APN和vWF、内皮素-1、NO水平,其中冠心病患者包括35例稳定型心绞痛(SAP)和25例急性冠状动脉综合征(ACS)的患者,比较各组间的差异有无统计学意义。结果冠心病患者血浆APN与NO均显著低于健康对照组,分别为(7.79±3.22)mg/L和(11.35±2.90)mg/L;(41.57±14.73)μmol/L和(67.51±18.69)μmol/L(P〈0.01),而血浆内皮素-1、vWF则显著高于健康对照组(81.65±23.07)ng/L和(52.17±13.65)ng/L;(119.3±54.59)%和(96.45±32.10)%(P〈0.05-0.01);其中ACS患者血浆APN、NO均显著低于SAP(6.66±3.10)mg/L vs(8.60±3.09)mg/L;(35.73±15.82)μmol/L vs(45.74±12.53)μmol/L(P〈0.05-0.01),而血浆内皮素-1、vWF均显著高于SAP(89.49±25.53)ng/L vs(76.05±19.65)ng/L;(136.63±53.94)%vs(106.95±52.35)%(P〈0.05)。血浆APN与内皮素-1呈显著负相关(r=-0.522,P〈0.01),与NO则呈显著正相关(r=0.415,P〈0.01),与vWF无明显相关性(r=-0.129,P〉0.05)。结论冠心病患者血浆APN、NO水平显著降低,而内皮素-1与vWF则与之相反,以ACS为著;血浆APN可能参与内皮功能的调节。Objective To investigate the plasma level of adiponectin,endothelin-1(ET-1) ,nitric oxide (NO) and yon Willebrand factor (vWF) in the patients with coronary heart disease and their relationship. Methods Plasma level of adiponectin and vWF, ET-1, NO in 28 healthy outpatients (healthy control group) and 60 patients with coronary heart disease (CHD) were detected by enzyme-linked immuneosorbent assay (ELISA), radioimmunoassay (RIA) and enzymatic assay with nitrate reductase respectively. Sixty patients with CHD were divided into two groups: stable angina pectoris (SAP) group ( n =35) and acute coronary syndrome (ACS) group ( n =25). Results Adiponectin and NO in the patients with CHD were significantly lower than those in healthy control group, respectively (7.79 ± 3.22) mg/L vs (11.35±2.90) mg/L; (41.57±14.73) μmol/L vs (67.51±18.69) μmol/L all P 〈 0. 01, while ET-1 and vWF were significantly higher, (81.65 ± 23.07) ng/L vs (52.17 ± 13.65) ng/L; (119.3 ± 54.59 )% vs (96.45 ± 32.10) % ( P 〈0.05-0.01), and the sameresults were observed between ACS group and SAP group, adiponectin (6.66 ±3.10) mg/L vs (8.60±3.09) mg/L( P 〈0.05) ; NO (35.73±15.82) μmol/L vs (45.74±12.53) μmol/L( P〈0. 01); ET-1(89. 49±25.53) ng/L vs (76. 05±19. 65) ng/L( P d0. 05); vWF(136. 63±53.94)% vs (106. 95± 52.35)%( P 〈0.05). The level of adiponectin was negatively correlated with that of ET-1 ( r=-0. 522, P d0.01) and positively correlated with that of NO in plasma ( r=0. 415, P〈0.01) ,while adiponectin was irrelated with vWF ( r=-0. 129, P〉0.05). Conclusion The plasma level of adiponectin and NO decreased significantly in the patients with CHD, while those of ET-1 and vWF increased significantly. Plasma adiponectin may play a role in the protection of endothelial function.
