加味四物汤治疗2型糖尿病合并无症状性高尿酸血症38例临床观察

摘　　要：目的观察中药方剂加味四物汤治疗糖尿病无症状性高尿酸血症患者的疗效。方法符合糖尿病及无症状性高尿酸血症诊断标准的患者76例,随机分为对照组38例,治疗组38例,对照组予科素亚50mg,每天1次;治疗组予加味四物汤,每天1次。疗程3个月。观察治疗前后血糖、胰岛素、血压、血胆固醇、甘油三酯、体重指数、血尿酸(SUA)及血肌酐,尿尿酸(UUA)及尿肌酐、尿pH值,尿微量白蛋白(U-mAlb),计算尿酸清除率、肌酐清除率、尿酸清除分数等。结果治疗组总有效率为81.58%,与对照组(60.53%)比较差异有显著性意义(P<0.01);治疗组SUA、尿pH值、UUA、24小时尿量等优于对照组(P<0.01或P<0.05);对照组U-mAlb、舒张压性优于治疗组(P<0.05)。结论加味四物汤对糖尿病无症状性高尿酸血症有显著疗效。Objective To study the therapeutic effect of Supplemented Siwu Decoction for type 2 diabetes with asymptomatic hyperuricemia. Methods Totally 76 patients according to the diagnostic code of diabetes and asymptomatic hyperuricemia were randomly and equally divided into two groups. Cozaar and Supplemented Siwu Decoction were given once a day to control and treatment groups respectively for 3 months. Fasting plasma glucose, insulin, blood pressure, cholesterol, triglyeride, body mass index, serum uric acid （SUA） ,serum creatinine,urine uric acid （UUA）, urine creatinine, urine pH value and urine microamount albumin （U-mAlb） were observed before and after treatment. Clearance of uric acid,clearance of creatinine and fraction of uric acid clearance were calculated as well. Results The total effective rates of treatment and control groups were 81.58% and 60. 53% respectively,resulting in significant difference （P^0.01）. Treatment group had better effect on SUA, urine pH value,UUA and 24-hour urine volume than control group （P〈0.01 or P〈0.05）. Control group had better effect on U-mAlb and diastolic blood pressure than treatment group（P〈0.05）. Conclusion Supplemented Siwu Decoction has obvious effect for diabetes with asymptomatic hyperuricemia.
