摘 要:目的 观察用组织工程方法修复中国青山羊胫骨大段骨与骨膜缺损3年后的远期效果。方法 中国青山羊3只,制备单侧胫骨20mm的骨与骨膜缺损模型,缺损内植入组织工程骨珊瑚羟基磷灰石/骨髓基质干细胞(CHAP/BMSCs),术后3年采用普通x线片、组织学观察以及血管铸型等方法对其进行远期观察检测,与健侧正常骨对照,观察其成骨及血管化效果。结果 X线片骨吸光度测定与健侧比较差异无统计学意义(P〉0.05);血管灌注后大体解剖观察示组织工程骨血管来源于周围软组织、髓腔血管及两端正常皮质骨;灌注后未脱钙骨磨片显示组织工程骨内微血管沿哈佛管和伏克曼管分布交织成网状,横切面磨片采用图像分析仪分析与正常骨血管相对面积比较差异无统计学意义(P〉0.05)。脱钙后的组织切片HE、硫堇染色示组织工程骨具有与正常骨一致的显微结构。结论 CHAP/BMSCs具有良好的修复山羊胫骨大段骨缺损能力,其远期显微结构和血管化效果与正常骨生理无异。Objective To observe the three-year effects of tissue engineering repair of 20 mm defects at caprine tibial shafts on the microstructure and vascularization of the engineered-bone. Methods The defects of 20 mm at 3 left hind tibias of 3 Chinese goats were reconstructed by filling coral hydroxyapatite (CHAP) plus induced bone marrow stromal cells (BMSCs) in 2004. Plain X-ray, histological observation and vascular casting were conducted in the 3 goats to observe the microstructure and vascularization of the experimental tibias in comparison with the normal tibias. Results The plain film determination showed no significant difference in optical density between the 2 sides( P 〉 0. 05) . Gross anatomic observation after vascular casting disclosed that the vessels of tissue-engineered bone grafts originated from the surrounding soft tissue, medullary cavity and normal cortical bone at the 2 ends. The ground sections of vascular casting samples without decalcification revealed that the capillaries of engineered-bone grafts went along the Haversion' s and Volkmann' s canals and interlaced into a net. The image-analyzer detected no significant difference between the samples of 2 sides in vaso-area ratio( P 〉 0. 05) . The demineralized sections of engineered-bone grafts by hematoxylin-eosin and thionine staining manifested very similar microstructure to that of the opposite. Conclusions CHAP / BMSCs can well repair the diaphyseal defects of caprine tibia with microstructure and vascularization similar to the normal bone. This study provides encouraging evidence for clinical application of tissue engineered bone.
