摘  要:【目的】研究应用益生菌治疗婴儿湿疹后,其外周血Th1/Th2细胞因子分泌细胞的变化及肠道菌群的改变,探讨益生菌作用的机理。【方法】60例重症湿疹患儿随机分成两组,益生菌组在对症治疗的同时服用双歧杆菌三联活菌,对照组仅对症治疗,不用任何微生态制剂。分别在益生菌治疗前及治疗后1月采指尖血,用ELISpot方法检测IFNγ,IL-4分泌细胞;同时采粪便标本,用荧光定量PCR的方法检测肠道菌群的变化。【结果】湿疹患儿在益生菌治疗30 d后,IL-4分泌细胞明显减少(P<0.05),IFNγ/IL-4比值明显升高(P<0.01),而IFNγ无明显变化。益生菌治疗前后,患儿肠道双歧杆菌,乳酸杆菌明显升高(P<0.01),而对照组无明显变化。【结论】益生菌能通过调节湿疹患儿肠道菌群,影响机体Th1/Th2平衡状态。[Objective] To examine the changes of Thl/Th2 cytokines secreting cells and intestinal microflora in infants with atopie eczema after probioties treatment, and discuss the mechanism of probioties thea. [ Methods] Sixty children with heavy eczema were enrolled and divided into two groups in random, the probioties group was given bifid triple viable capsules and the control group never received any probioties besides the routine treatment. Before and after probioties therapy, finger tip blood was collected and ELIspot was applied to detect the cells of sereeting eytokines IFNγ and IL-4 , and Real Time PCR were used for quantitative detection of mieroflora species in their feces. [Results] In the probioties group after a 30-day therapy, the cells of IL-4 secreting were significantly decreased (P〈0.05),and the ratio of IFN/IL- 4 was rised. But there was no change in IFNγ secreting cells. Moreover, after therapy, the proportion of bifidobaeteri and laetobaeilli were increased significantly in the probioties group (P〈0. 01), while there was no change in the control group. [Conclusion] Probioties can affect the balance of Thl/Th2 in infants with eczema, which might through the regulation of intestinal mieroflora.
