摘 要:目的:探讨针灸大肠下合穴上巨虚对溃疡性结肠炎(UC)大鼠肠黏膜形态学及生长激素(GH)、胰岛素样生长因子-I(IGF-I)表达的作用。方法:采用免疫法加局部刺激制备UC大鼠模型,随机将大鼠分为模型组、隔药灸组、电针组,并与正常大鼠作对照。隔药灸组、电针组选取上巨虚穴分别进行隔药灸、电针治疗,连续治疗14次。治疗结束后采用光、电镜观察肠黏膜形态学变化;应用免疫组化法检测结肠GH、IGF-I蛋白表达。结果:UC大鼠结肠光镜观察可见溃疡形成和炎症,电镜观察可见结肠黏膜上皮细胞绒毛脱落短小缺损,胞膜损伤,线粒体肿胀,嵴消失,高尔基体囊泡较少,杯状细胞膜严重受损,胞内没有黏液颗粒;结肠GH、IGF-I蛋白表达增高。隔药灸组、电针组经过治疗后,结肠黏膜病理损伤减轻;结肠GH、IGF-I蛋白表达降低,趋于正常。结论:隔药灸、电针上巨虚穴均能明显改善UC大鼠结肠黏膜病理损伤,保护肠黏膜,促进肠黏膜机械屏障的修复;UC大鼠结肠GH、IGF-I表达增高,隔药灸、电针上巨虚穴均能促进结肠异常增高的GH、IGF-I的表达趋于正常。Objective :To investigate the intestinal mucosa morphological effects of acupuncture and moxibustion on Shangjuxu ( ST 37, the lower confluent acupoint of large intestine) in ulcerative colitis ( UC ) rats and the change of Growth hormone(GH) and Insulin-like growth factor-I(IGF-I) of the UC rats'colons after acupuncture and moxibustion. Methods :The model of the UC rats was formed by immunological method plus local stimulation. Rats were divided into model group,herbs-partition moxibustion group and electro-acupuncture group randomly, meanwhile eonstrasted with normal group. Rats in experimental groups were treated for 14 days. The changes of intestinal mueosa morphology were observed by light and electron microscopy. The expressions of GH and IGF-I proteins were determined by immunohistochemical method. Results : Helcosis and inflammation could be seen under light microscope; Exfoliation, shortness and lack of the intestinal epithelial Villi, loss and damage of epithelial cell membrane, swell of mitochondria, disappearance of cristae,less of golgiosome vesicle, severe damage of goblet cell membrane, and no muciage granule inside the cell could all be seen under electron microscope. The protein expressions of beth GH and IGF-I were higher. After treatment of herbs-partition moxibustion and electro-acupuncture the pathological damnification of intestinal mucosa were alleviated, and the GH and IGF-I protein expressions of the colons became lower, turned out to be normal. Conclusions :Both herbs-partition moxibustion and electron-acupunture on ST 37 could significantly improve the pathological damnification of the UC rats' co- Ions, protect the intestinal mucosa, and accelerate the recover of intestinal mucosal barrier. Herbs-partition moxibustion and electron-acupuncture on ST 37 could regulate the high expression of intestinal GH and IGF-I to normal.
