彩色实时三维超声心动图在室间隔缺损介入治疗中的应用价值

摘　　要：目的：探讨彩色实时三维超声心动图在室间隔缺损介入治疗中的应用价值。方法：对36例先天性室间隔缺损患者在实时三维超声心动图及彩色实时三维超声心动图监测下，经心导管行室间隔缺损封堵术。结果：34例室间隔缺损患者术前经胸超声心动图和彩色实时三维超声心动图估测室间隔缺损的最大径与造影估测的大小的相关性分别为r=0.97（P〉0.05）及r=0.99（P〉0.05），彩色实时三维超声心动图能立体显示室间隔缺损的血流起源、方向和立体空间分布，能准确引导室间隔缺损介入手术。结论：彩色实时三维超声心动图对室间隔缺损封堵术前的诊断、疗效评估和预后评价方面具有重要的临床应用价值和应用前景。Objective To investigate the value of color real-time three-dimensional echocardiography （RT-3DE） in transcatheter clousure of ventricular septal defect（VSD）. Methods Philips Sonos 7500 and IE33 with a X4 probe was used. Thirty six patients[23 male,13 female,mean age （20. 7 ±10. 6）years] with ventricular septal defect underwent transcatheter closure under the guidance of RT-3DE and color RT-3DE. Results Before occlusion, the largest diastolic size measured by transthoracic echocardiography and color RT-3DE compared with those measured by left ventricular opacification,its correlation were separately r = 0.97, P 〉0.05 and r = 0.99, P〉0.05. Color RT-3DE can display clearly origin of blood flow, direction and its spatial relation to the surrounding tissue,guiding the transcather closure procedure. Conclusions Color RT-3DE provides for rapid and accurate methods being monitored during guiding the transcather closure procedure. It has important clinical value and perspective on the clinical diagnosis and the evaluation of therapeutic effect and prognostic appraise for ventricular septal defects.
