恶性肿瘤患者125例肠外与肠内营养支持的护理研究

摘　　要：目的探讨营养支持在癌性营养不良治疗中的合理性及有效性,以及对改善癌症患者营养不良、免疫状况及生活质量的作用,并制定相关护理常规。方法回顾性分析行肠内或肠外营养支持的125例恶性肿瘤患者的临床资料,通过检测血清生化指标、免疫指标及生活质量评分来评价患者的营养状况。结果125例患者中86例行肠外营养支持,94例行肠内营养支持,其中肠内与肠外联合营养支持55例。营养支持时间为10～214d,平均(28.7±12.6)d,治疗有效率达到78.4%(98/125)。营养支持后患者血清白蛋白浓度、BMI、KPS评分显著增加,血清CRP浓度有显著下降(P<0.05)。肠外营养并发症发生率为7.0%(6/86),肠内营养并发症发生率为10.6%(10/94),所有并发症经处理后均治愈。结论营养支持在癌性营养不良的治疗中是安全、有效的,可以改善患者营养状况,提高生活质量,改善预后。Objective To study the rationality and validity of nutrition support in treatment of patients with cancerous malnutrition and the effect of nutrition support on improving the nutritional status, immune function and quality of life of cancer patient, and to establish the related nursing routine. Methods The clinical data of 125 cancer patients receiving enteral or parenteral nutrition was retrospectively analyzed. The biochemical indicators, immune function and quality of life score were determined to evaluate the nutritional status of the patients. Results Eighty-six patients received parenteral nutrition,94 patients received enteral nutrition, and 55 patients received both enteral combined parenteral nutrition. Duration of nutritional support was 10-214 days （28. 7±12.6） ,and the effective rate of nutritional support was 78.4%（98/125）. The levels of serum albumin,BMI and KPS scores were significantly increased（ P 〈0.05） and the serum CRP level was significantly decreased （ P 〈 0. 05）. Complications of parenteral nutrition and enteral nutrition were 7.0% （6/86） and 10.6% （10/94）, respectively; and were treated successfully. Conclusion Nutritional support is safe and effective in treatment of cancerous malnutrition and can improve the nutrition status, quality of life and prognosis of cancer patients.
