社区COPD的流行病学调查及干预性防治的疗效观察

摘　　要：目的了解上海市社区成年居民慢性阻塞性肺疾病(COPD)患病率,通过早期综合干预措施,减少COPD急性发作的频率,提高患者的生命质量。方法在打浦及东南的二个居民服务点,对常居30岁以上的3450名成年居民进行COPD患病率的流行病学调查,随机选取其中40例患者进行为期2年的综合干预(治疗组),另一组40例患者根据患者需要维持一般治疗(对照组)。2年后分别采用肺功能和生命质量测评表进行评估。结果该地区30岁以上常居者3116人,COPD患者113例,总患病率为3.6%(113/3116)。2年后干预组与对照组比较,干预组生命质量评分低于对照组,差异有显著意义(P<0.05)。结论COPD在社区患病率较高,早期综合干预治疗可能是阻止疾病进展的关健环节。Objective Discover the adults morbidity of chronic obstructive pulmonary disease （COPD） in urban districts in Shanghai and decrease the frequency of acute exacerbation of COPD through early systemic interference. Methods Epidemiological investigation was adapted in 3116 adults citizen in two residents service point from two community （ Dongnan and Dapu）. The methods of investigation include inquiring, filling in epidemiological question paper and performing pulmonary function tests.. Systemic interference for two years was taken in 40 patients chosen from diagnosed COPD randomly, （ interference group）, Another 40 patients also chosen from diagnosed COPD randomly, （controlled group） were only given general treatment for two years according to patients＇demand, Evaluation was made based on pulmonary function tests and llfe quality paper after two years, Results Total effective examined persons are 3116 and 113 patients of COPD are detected. Morbidity of COPD is 3, 6% （113/3116）, Compared to controlled group, interference group has lower decreasing velocity in FEV1 and FVC （ P 〈 0. 05 ） ; better marks in life quality, （ P 〈 0. 05 ）, Conclusions The morbidity of COPD in urban districts in shanghai is also higher, Early systemic interference may be the key point in improving this disease,
