多普勒超声心动图对慢型克山病诊断的综合评价

摘　　要：用Ｍ型２－ＤＥ及多普勒超声对３０例经临床确诊的慢型克山病患者进行综合评价，发现克山病的多普勒超声心动图具有下列特征性改变：１．各房室腔普遍扩大，并以左侧心腔为甚；２．室壁活动多呈弥漫性减弱（占９３％）；３．多瓣膜返流；４．收缩功能的各项参数指标明显降低，而舒张功能的指标在无ＭＲ病例与对照组比较有明显差异（Ｐ＜０．０１），从充盈期血流改变来看提示快速流入血流亦受阻，证明慢型克山病的心脏舒张功能亦异常。表明多普勒超声心动图能准确地反映克山病患者心脏的病理生理及血流动力学改变，为克山病诊断提供可靠依据。Thirty clinically confirmed Keshan disease patients were comprehensive estimated by M type and 2 DE Doppler echocardiography.Results show that there are characteristic changes in the Doppler echocardiography of Keshan disease patients.(1) Patient's heart is widely enlarged,especially in the left cavity.(2) The motion of ventricular wall is decreased widely.(3) Valvular regurgitation could be observed in most cases.(4) The parameters of ventricular systolic function are generally lower than those of normal,however,no difference between the parameters of ventricular diastolic function of normal persons and patients could be seen(p>0.05). Our results suggested that Doppler echocardiography can accurately reflect the changes of pathology and hemodynamics of Keshan disease.
