摘 要:目的本研究拟通过在线式测定预备脱机病人低水平压力支持通气(pressure support ventilation,PSV)时气道阻断压(P0.1),以公式Pi=5×P0.1×Ti计算平均吸气压(Pi),比较Pi与食管压力(Peso)的相关性与一致性。方法12例预备脱机的机械通气病人,通过插入食管囊管监测食管压力(Peso)。病人在PSV水平分别为5cmH2O通气15min。按呼气末暂停键测定P0.1。通过公式计算Pi,分别分析P0.1以及Pi与Peso之间的相关性。结果在PSV水平分别为5cmH2O时,P0.1与Peso之间以及Pi与Peso之间的相关系数分别为0.881和0.906。结论预备脱机病人在较低的PSV水平时,在线式测定的P0.1与Peso之间以及Pi与Peso之间均具有良好的相关性;Pi较P0.1更适合用于PSV通气时呼吸努力的无创性评估。Objective To investigate the correlation between esophageal pressure (Peso) and mean inspiratory pressure (Pi) which was calculated from formula Pi =5 × P0.1 × Ti, in which P0.1 was measured online during low level pressure support ventilation (PSV) in patients weaning from mechanical ventilation. Methods 12 mechanically ventilated patients were enrolled in the study. Esophageal pressure (Peso) was monitored through esophageal balloon inserted. Patients were ventilated at 5mH2O for 15 minutes. P0.1 was measured by means of the end - expiratory hold function of the experimental ventilator. Pi was calculated from Pi =5 × P0.1 × Ti. Correlation analyses were performed between P0.1 and Peso as well as between Pi and Peso. Results At 5 cmH2O level of PSV, the correlation coefficient between P0.1 and Peso was 0. 881, and that between Pi and Peso was 0.906. Conclusion At relatively lower PSV level, there was good correlation between P0.1 and Peso as well as between Pi and Peso. Pi was more suitable than P0.1 for noninvasive assessment of respiratory effort in PSV.
