电针刺联合帕罗西汀治疗持续性躯体形式疼痛障碍的临床研究

摘　　要：目的研究电针刺联合帕罗西汀治疗持续性躯体形式疼痛障碍的疗效。方法将63例持续性躯体形式疼痛障碍的患者随机分为电针组(B组:电针刺治疗)、帕罗西汀组(C组:帕罗西汀治疗)和研究组(A组:电针刺联合帕罗西汀治疗)。在治疗前及治疗1、2、4、8周末各评定1次简易McGill疼痛量表(SF-MPQ)、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)和治疗不良反应量表(TESS)。结果治疗后3组患者疼痛缓解疗效间差别无统计学意义(P>0.05)。A组的PRI总分、VAS得分、PPI得分、HAMD总分和HAMA总分,从治疗第1周末起与治疗前比较,差别有统计学意义(P<0.05),C组从治疗第2周末起与治疗前比较,差别也有统计学意义(P<0.01);B组的PRI总分、VAS得分从治疗第1周末起与治疗前比较,差别有统计学意义(P<0.05),而PPI得分、HAMD总分、HAMA总分从治疗第2周末起与治疗前比较,差别有统计学意义(P<0.01)。3组患者治疗不良反应少。结论电针刺联合帕罗西汀治疗持续性躯体形式疼痛障碍可以显著控制疼痛,改善患者不良情绪,且比帕罗西汀单独治疗起效快。Objective To validate the clinical effectiveness of electric - acupuncture combined with paroxetine in the treatment of durative somatoform pain disorder. Methods 63 cases of the durative somatoform pain disorder patients were randomly divided into three groups. Group A ： electric - acupuncture combined with paroxetine group ; Group B ： electric - acupuncture group ; Group C ： paroxetine group. Before the treatment and on the 1, 2 , 4 and 8 week ofter the treatment, pain, dumps, anxiety and the adverse effects were measured according to Mc Gill, SF - MPQ, HAMD, HAMA and TESS respectively. Results There were no significant differences of pain relif rate between three groups. On the 1, 2, 4 and 8 week after treatment, there were significant differences of the score of PRI, VAS, PPI, HAMD and HAMA compared with pre- treatment in group A （ P 〈0. 01）. In group C, on the days of 2, 4 and 8 week after treatment, there were significant differences of the score of PRI, VAS, PPI, HAMD and HAMA compared with pre - treatment （ P 〈0. 01）. In group B, on the days of 1, 2, 4 and 8 week after treatment, there were significant differences of the score of PRI, and VAS compared with pre - treatment （ P 〈 0.01 ）, as for PPI, HAMD and HAMA, There were significant differences on the days of 2, 4 and 8 week compared with pre - treatment （ P 〈 0. 01 ）. Conclusion Electric - acupuncture combined with paroxetine is effective in the treatment of durative somatoform pain disorder.
