摘 要:目的探讨应用锁定加压接骨板(LCP)微创治疗下肢长骨粉碎性骨折的疗效。方法2004年12月~2006年10月,采用LCP内固定治疗42例下肢长管状骨粉碎性骨折患者,其中股骨干骨折18例,胫骨干骨折24例。通过透视下闭合复位,在骨折两端骨皮质完整处行小切口,将钛板导入,不显露骨折断端。术后定期随访复查X线片,观察骨折愈合情况。结果39例患者术后经平均16个月(4~22个月)随访,均获骨性愈合,平均愈合时间为5.5个月,患肢功能满意。根据Johner-wruhs评分评估患者膝、踝关节功能:优29例,良6例,可4例,优良率为89.7%。所有患者术后无一例发生并发症。结论LCP治疗下肢长管状骨粉碎性骨折具有微创、操作简单、固定牢固的特点,对骨质疏松、髓腔较细或干骺端骨折患者的治疗比普通钢板及交锁髓内钉更具有优势。Objective To investigate the effect of treating comminuted long bone fractures of lower extremity with minimally invasive locking compression plate (LCP). Methods From December 2004 to October 2006, 42 cases of comminuted long bone fractures of lower extremity were treated with LCP internal fixation. Closed reduction was performed under fluoroscopy. Two small incisions were made at the proximal and distal ends of fractures where the cortical bone was intact. The LCP was inserted without exposure of fracture fragments. X-ray follow-ups were made after operation to observe bone healing. Results Thirty-nine cases were followed up for an average of 16 months(range, 4 to 22 months). They all got bone healing in an average time of 5.5 months. The patients were satisfied with functional recovery. According to the rating scale of Johner-wruhs, 29 cases got excellent results, 6 cases good resulds, and 4 cases fair results. The excellent and good rate was 89.7%. No complications occurred. Conclusions In treatment of comminuted long bone fractures of lower extremity, LCP internal fixation is minimally invasive, simple and stable. It is good especially for the patients with osteoporosis, small medullary cavity or metaphyseal fracture.
