摘　　要：目的观察机械通气撤离(简称撤机)过程中胃黏膜代谢指标的变化及其对撤机的预测作用。方法利用胃张力测定仪监测86例患者在撤机过程中胃黏膜pH值(pHi)、胃黏膜内二氧化碳分压(PgCO2)、胃黏膜内-动脉血二氧化碳分压差[P(g-a)CO2]等指标的变化。根据结果分为成功组、失败组、死亡组。各指标采用单因素方差分析、双因素方差分析,组间两两比较采用q检验。结果3组患者各阶段的胃黏膜代谢指标差异均有统计学意义(P<0.05)。结论胃黏膜代谢指标在预测撤机方面有重要意义。Objective Observe the changes of the metabolic indexes of gastric mucosal in the weaning process of mechanical ventilation and its forecast affects.Methods 86 patients with acute respiratory failure were examined by gastric tonometer during the process of mechanical ventilation and weaning.The changes of arterial blood gas （ABG） indexes, gastric intramucosal pH （pHi）, gastric intramu cosal CO2 pressure （PgCO2）, and gastric intramucosal - arterial CO2 pressure （P（g-a）CO2） had been obtained by examining.According to the outcome of weaning from mechanical ventilation,patients were divided into the success group,the failure group and the dead group. Analyze the indexes before the weaning process of mechanical ventilation with single factor variance analysis, the indexes after the weaning process of mechanical ventilation with double factor variance analysis, and the two indexes each between the groups with q-examines.Results In each periods of the weaning process of mechanical ventilation, the differences of the metabolic indexes of gastric mucosal among the three groups had statistics significance （P〈0.05）.Conclusion the metabolic indexes of gastric mucosal are very important in predicting tire mechanical ventilation weaning,
