摘　　要：目的通过分析老年人左室肥厚(LVH)的影响因素,来寻找延缓或逆转LVH的途径。方法对年龄在65～91[76.00(5.00)]岁的312名老年人进行问卷调查、体格检查、实验室检查、心脏超声检测和24h动态血压检测,运用多元线性回归分析方法分析影响老年人LVH的因素。结果老年人LVH的影响变量有24h收缩压(β=0.325,P=0.024)、血钾(β=-12.448,P=0.013)、血尿酸(β=0.058,P=0.027)、载脂蛋白A(β=-0.113,P=0.020)和载脂蛋白B(β=-0.199,P=0.030)。结论降低24h收缩压水平和尿酸浓度、提高血钾、载脂蛋白A的浓度,是延缓或逆转左室肥厚的重要途径。载脂蛋白B与左室肥厚的关系有待临床进一步研究。Objective Left ventrieular hypertrophy （ LVH ） is a strong predictive factor of cardiovascular morbidity and mortality, however factors associated with LVH have seldom been studied. The purpose of the study is to probe into the ways to postpone or reverse LVH by means of analysing its factors associated in the old. Methods A cross - sectional survey was conducted comprising a sample of 312 subjects aged 65 -91 [76.00（5.00） ] years. The questionnaire contains items on physical examination ,laboratory measurement,echocardiography and 24h ambulatory blood pressure monitoring. Univariate analyses and multivariate linear regression analysis were applied to discover factors associated with Left ventrieular mass index（LVMI）. Results The finding of the study showed that factors associated with LVMI in the old included 24h systolic blood pressure（SBP） （β = 0.325, P = 0.024 ） ,blood potassium （β= - 12. 448, P = 0.013 ） ,blood uric acid（β = 0.058,P = 0.027 ）, apolipoprotein A （β= - 0.113, P = 0. 020 ） and apolipoprotein B （β = - 0. 199, P = 0. 030 ）. Conclusions The important ways to postpone or reverse LVH are both to decrease the level of 24 SBP and the concentration of blood uric acid and to increase the concentration of the blood potassium and apolipoprotein A. The relation between apolipoprotein B and LVH yet is to be studied.
