女性心力衰竭患者血清瘦素水平及其与胰岛素水平的关系

摘　　要：目的:探讨女性心力衰竭患者血清瘦素水平改变及其与胰岛素水平间的关系。方法:入选女性心力衰竭患者66例,其中绝经前患者29例,绝经后37例;男性心衰患者56例;对照组正常女性30名,测定所有研究对象血清瘦素、血糖、胰岛素水平,并分别测量体重指数(BMI)和腰围/臀围比值(WHR)。结果:女性心衰组血清瘦素水平为[(166.78±25.76)pg/ml]显著高于同龄女性正常组[(90.60±20.90)pg/ml]和同龄男性心衰患者[(61.38±29.04)pg/ml](P<0.05,P<0.01),且与BMI、WHR及空腹胰岛素浓度呈正相关(r=0.61,0.52,0.64,P<0.01)。绝经后心衰患者瘦素浓度[(163.99±25.20)pg/ml]较绝经前患者[(170.25±26.30)pg/ml]略低,但两者间差异无统计学意义。女性心衰患者胰岛素水平[(25.87±5.78)mU/L]较正常对照组[(19.21±6.10)mU/L]明显增高(P<0.05),但两者血糖水平并无差异。结论:女性心力衰竭患者血清瘦素水平显著升高,并与胰岛素水平呈正相关。Objective To discuss the change of plasma leptin level and its relationship with insulin level in female patients with congestive heart failure. Methods Serum levels of leptin and insulin were measured at a fasting state in 122 consecutive patients with congestive heart failure, including 66 females （29 premenopause and 37 postmenopause） and 56 males. Thirty normal females were served as controls. Body mass index（BMI） and waist hip ratio（WHR） were also measured in all subjects. Results Serum levels of leptin were significantly higher in female patients [ （166.78±25.76） pg/ml]than those in normal females [ （90.60±20.90） pg/ml] and male patients [ （61.38±29.04） pg/ml] （P〈0.05,P〈0.01）, which were positively correlated with BMI, WHR and serum levels of insulin [r=0.61,0.52,0.64,P〈0.01 ）. Serum levels of leptin were slightly lower in postmenopausal female patients[ （163.99±25.20） pg/ml]than those in premenopausal female patients[ （170.25±26.30） pg/ml]without significant difference. Serum levels of insulin were greatly increased in female pa- tients[ （25.87±5.78） mU/L]compared with those in the controls[ （19.21±6.10） mU/L] （P〈0.05）,hut no difference on blood glucose levels was found between the two groups. Conclusions Serum levels of leptin greatly increase in female patients with congestive heart failure, which are positively correlated with serum levels of insulin.
