摘 要:目的:评价脐血乙型肝炎病毒(hepatitis Bvirus,HBV)核酸载量检测结果。方法:通过建立扩增效率分析法,评价脐血进行HBV-DNA定量检测的有效性;通过比较脐血HBV血清学标志物的检测结果与脐血标本HBV-DNA定量检测结果,评价检测方法。结果:HBV血清学标志物检测结果为大三阳,即HBsAg(+)、HBeAg(+)和HBcAb(+)的乙型病毒性肝炎产妇,其新生儿脐血标本在HBV-DNA定量检测中,20例中有13例(65%)发生扩增,扩增效率分析与其他血清标本差异无显著性;该20例脐血标本的HBV血清学标志物检测结果HBcAb(+)12例(60%),HBeAg(+)11例(55%),而其中HBsAg(+)仅1例(5%)。结论:脐血标本可用于检测HBV核酸载量。Objective To evaluate the outcomes of quantitative detection of HBV-DNA in umbilical cord blood (UCB). Methods The effects of quantitative detection of HBV-DNA were evaluated through an established analysis method for amplification efficiency, and the clinical significance of this detection was assessed by comparing its outcomes with the detectable HBV serological markers in UCB. Results 20 UCB samples from the newborns whose mothers were triply positive for HBsAg, HBeAg, and HbcAb were detected using quantitative detection of HBV-DNA. 13 of the 20 samples (65%) showed positive amplification, with no significant difference in the amplification efficiency as compared to other serum samples. 12 of the samples (60%) were HbcAb positive, 11 (55%) HBeAg positive, and only 1 (5%) was HBsAg positive. Conclusion Umbilical cord blood can be used for quantitative detection of HBV-DNA whose outcomes can be a more sensitive and definite indicator for the neonates vertically infected with HBV.
