产科出血218例死亡原因分析

摘　　要：目的 探讨安徽省孕产妇死亡中产科出血死亡的相关因素及死亡率下降的有效干预措施。方法 通过省级孕产妇死亡监测点收集上报孕产妇死亡个案等监测资料．结合评审，对2000年至2005年安徽省孕产妇死亡监测点的218例产科出血孕产妇死亡进行死因及相关因素分析。结果 218例产科出血死亡孕产妇死因构成依次为宫缩乏力占44.50％，胎盘残留占16．51％，子宫破裂占13．76％；孕产妇死亡还与文化程度、产前接受保健服务程度、基层网络建设、保健服务人员的知识水平等因素有关。结论 加强各级产科建设，落实孕产妇系统管理，提高住院分娩，提高业务人员服务能力，提高妇女的自我保健意识．是减少产科出血造成的孕产妇死亡的主要措施。Objective To discuss relevant factors of the deaths of the pregnant and lying-in women due to obstetric bleeding in Anhui province and the effective intervening measures to reduce the fatallty-rate. Methods The materials were evaluated about the cases of deaths of the pregnant and lying-in women collected and reported by monitoring offices in Anhui province,and the causes and relevant factors of 218 deaths of the pregnant and lylng-in women were analyzed due to obstetric bleeding reported by monitoring offices between 2000 -2005 in Anhui province. Results The causes of 218 deaths due to obstetric bleeding were：44.5% of the deaths was caused by insufficient constriction of wombs, 16.51% was caused by remaining placentas, and 13.76 % was caused by ruptures of wombs; The deaths were also related to educational level, the quality of health care services provided before giving birth,the construction of the prilimilary health care system, professional knowledge of the health care providers,and etc. Conclusion In order to reduce the deaths of the pregnant and lying-in women casued by obstetric bleeding,it is essential to enhance the building of obstetric agencies of different levels, carry out the systematic management of the pregnant and lying-in women, encourage more women to give birth in hospitals, promote the professional capacity of the medical stuff, and strength women＇s self-awareness of health care.
