摘 要:目的评价运动负荷前后超声心动图(ESE)、运动心电图(运动ECG)对冠心病的诊断价值。方法对46例冠脉造影阳性(冠心病组)和冠脉造影阴性22例(正常组)的患者进行ESE与运动ECG的同步运动实验。结果运动可诱发明显的血流动力学变化,检测心肌缺血的敏感性、特异性和准确性高,分别为84.78%、91.6%和83.7%。两组阳性率对比经χ2检验有显著差异(P<0.01)。结论ESE对心肌缺血检出的敏感性明显高于运动ECG,ESE能够在心电图无改变或改变之前检出因缺血加重所致的心肌收缩力障碍。Objective To assess the diagnostic value of coronary artery disease (CAD) by exercise stress echocardiography (ESE) and exercise electrocardiogram. Methods 46 cases of coronary angiographic positive patients (CAD group) and 22caces negative ones (normal group) were examined by the synchronous stress exercise stress echocardiography. Exercise electrocardiogram and their positive tests of the tests were compared. Results ESE evoked higher hemodyrmmic changes in myocardium, its sensitivity, specificity and accuracy in detecting myocardial Ischemia were 84.78% ,91.6% 和 83. 7% respectively. Conclusion The sensitivity of ESE for myocardial ischemia is significantly higher than that in exercise ECG. The ESE can detect the disfunction of contraction of heart muscles caused by myocardial ischemia before the abnormal ECG or ECG without changes.
