摘 要:目的:探讨CYP17基因多态性和雌激素暴露因素交互作用与乳腺癌易感的关系。方法:应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)技术检测武汉市430例正常女性和210例女性乳腺癌患者雌激素代谢酶CYP17基因多态性;采用非条件logistic回归模型对CYP17基因多态性、雌激素暴露因素进行单因素和多因素分析。结果:乳腺癌患者雌激素代谢酶CYP17 A2/A2基因型频率为6.7%,显著高于对照组的2.4%(P<0.05),CYP17变异等位基因A2病例组的频率为16.2%,亦明显高于对照组10.6%(P<0.05);logistic回归分析显示:CYP17 A2/A2基因型与乳腺癌的发生具有统计学关联(P=0.013),CYP17基因多态性和雌激素暴露因素交互作用对乳腺癌的发病具有统计学意义的关联。结论:雌激素代谢酶CYP17基因多态性、雌激素暴露有关因素及其交互作用与乳腺癌易感有相关性。Objective: To investigate the interation between polymorphism in estrogen metabolizing genes CYP17 and estrogen exposure and its relation with breast cancer susceptibility. Methods: PCR- base restriction fragment length polymorphism (PCR-RFLP) assay was used to detect the single nucleotide polymorphism of CYP17 and CYP19 in 213 breast cancer cases and 430 matched controls. Univariate unconditional logistic regression was used to analyze the effect of the collected factors on the risk of the breast cancer. Results: CYP17 A2/A2 genotype was found in 6.7% of breast cancer cases, which was significantly higher (P〈0.05) than that in controls (2.4%) the frequency of A2 allele of CYP17 was 16.2% in breast cancer cases, which was significantly higher (P〈0.05) than that in controls too (10.6%). Results from univariate unconditional logistic regression analysis showed that there was a positive correlation between CYP17 A2/A2 genotype and the breast cancer susceptibility, there was a statistically significant correlation be tween the interaction of polymorphism in estrogen metabolizing genes CYP17 and estrogen exposure(dysmenorrheal, times of pregnancy, duration of lactation and menopausal age) and the breast cancer susceptibility too. Conclusion. There was a statistically significant correlation between the polymorphism in estrogen metabolizing genes CYP17 and estrogen exposure, and between their interaction and the breast cancer susceptibility.
