摘 要:目的:探讨切除不同长度Ranvier区对骺板发育的影响。方法:选用封闭群系新西兰大耳白兔48只,随机分为2个实验组,每组24只,采用切除长度分别为4 mm和12 mm长的胫骨近端Ranvier区,于术后1,4,8及12周处死实验兔。并从大体、X线摄片和组织学方面观察骺板发育情况。结果:4 mm和12 mm组骺板损伤周长百分率分别为(12.50±0.09)%和(37.50±0.20)%。术后8,12周12 mm组实验侧胫骨较对照侧短缩(0.97±0.49),(2.78±0.46)mm(P<0.05),而4 mm组未出现骨生长抑制。组织学观察发现术后1周软骨细胞增生带明显增厚,4周骨桥开始形成,8周骨桥成熟,12周成熟的横跨骺板的骨桥明显增厚。结论:Ranvier区切除造成骺板边缘损伤与切除长度有关,超过骺板周长约12.5%就可能导致骨生长抑制,Ranvier区在骺板横向生长中具有抑制作用。Objective: To explore the effect of excising various scale of the Ranvie's groove on the development of epiphyseal plate in circumference. Methods: The proximal growth plates of the tibias were exposed in young rabbits, and then the Ranvier's grooves and fibrous covers were removed at the length of 4 mm and 12 mm respectively. The percentage of excised area was estimated and the epiphyseal were anatomically and histologically evaluated. Results: Percentages of injured growth plates were (12.5±0. 09) and (37.50±0.20) % respectively in the 4 mm and 12 mm group. After 8,12 weeks, shortening of tibia in the 12 mm group was (0.97±0.49), (2.78±0.46) mm and enlarging tibia angle(10. 867±1. 354), (16. 608±1. 080) degree, whereas none was observed in the 4 mm group (P〈0. 05). Histological studies showed that proliferation zone became thicker, the bony-bridge began to form four weeks postoperatively. Bone bridge spanned the growth the growth plate on the 8th week and enlarged in size on the 12th week, respectively. In the 12 mm group, proliferation zone next to the bony-bridges became thinner and no growth retardation was found in 4 mm group. Conclusion: Epiphyseal plate retardation secondary to a excising injury of the Ranvier's groove is related to the percentage of the injured scale. If the scale〉 12.5%, the bone growth in width may be retarded. Ranvier's groove is responsible for inhibition the bone growth in circumference.
