软组织严重损伤胫腓骨开放骨折的分期治疗

摘　　要：目的探讨开放性软组织床重度损伤的双侧胫腓骨骨折的分期治疗。方法回顾性分析1999年1月至2005年1月我院收治的G ustilo分型ⅢB开放性软组织床重度损伤的双侧胫腓骨粉碎骨折病人40例,男36例,女4例,年龄18-55岁,平均33岁,进行分期治疗。采用膝关节、踝关节评分评价疗效。结果随访1.5-5年,平均2.6年。膝关节评分优（90-100分）4例,良（80-89分）5例,可（70-79分）22例,差（60-69分）4例。踝关节评分优（90-100分）6例,良（80-89分）23例,可（70-79分）4例,差（60-69分）2例。5例截肢。结论分期治疗开放性软组织床重度损伤的双侧胫腓骨骨折可明显提高患肢的存活率及功能恢复。Objective The staging treatment of double open tibia and fibula fractures with serious soft tissue bed damage. Methods Forty cases （36 male and 4 female with average age of 33 years ranging from 18 to 55 years） with double open tibia and fibula fractures with serious soft tissue bed damage were treated by the staging teatment, All the patients are Gustilo＇s ⅢB type. Using knee evaluation and ankle evaluation to evaluate the extremity function. Results All patients were followed-up for 2-5 years, Knee evaluation excellent（90-100） 4 cases, good （80-89） 5 cases, fair （70-79） 22 cases,bad （60-69） 4 cases. 5 cases were amputated extremity. The outcome is satisfactory. Conclusion The staging treatment of double tibia and fibula fractures with heavy soft tissue bed damage can improve the survival rate and the functional recovery.
