能量多普勒血流显像对隐匿性颈淋巴结转移癌的诊断价值

摘　　要：目的探讨能量多普勒血流显像(PDI)对隐匿性颈淋巴结转移癌的诊断价值。方法43例行颈淋巴结清扫术的头颈癌患者,术前接受颈部PDI检查,选择短轴最大直径<10mm的淋巴结64个,记录其PDI结果,术后标本经病理检查确定有无癌转移。结果良性淋巴结的血流分布以中央型为主,血流强度多为Ⅰ级,恶性淋巴结则以周边型或混合型为主,血流强度多为Ⅱ级以上,收缩期血流速度(PSA)、左心室舒张期容积(EDV)、血管阻力指数(RI)、搏动指数(PI)在良、恶性淋巴结间无显著性差异。结论PDI对于隐匿性颈淋巴结转移癌具有重要的诊断价值,当淋巴结周边出现血流信号时多提示恶性病变。Objective To explore the diagnostic value of power Doppler imaging（PDI）in occult cervical lymph nodes metastases. Methods The PDI date of 64 pathologically verified cervical nodes（30 benign,34 malignant）were analyzed retrospectively;these nodes obtained from 43 cases of head and neck cancer and measured less than 10mm in minimal axial diameter. Results Benign cervical lymph nodes showed characteristic features of central vascular pattern and grade I vascularity,while malig-nant cervical lymph nodes showed characteristic features of peripheral or mixed vascular pattern and grade Ⅱ or grade Ⅲ vascularity. There were no difference between benign and malignant cervical lymph nodes about the blood flow velocity and vascular resistance. Conclusion PDI has important value in the diagnosis of the occult cervical lymph nodes metastases, the presence of peripheral blood flow sign in cervical lymph nodes is likely to indicate malignant nodes.
