摘 要:研究采用18只接受纯种近段空肠异体移植的Wistar大鼠,随机分组,分别给于常规全肠外营养(n=8)和含3%丙氨酰-谷氨酰胺二肽的全肠外营养(n=10),持续10天.应用光镜、电镜和组织化学等技术对两组大鼠的移植小肠进行形态学的对比研究.结果显示:谷氨酰胺组移植小肠的粘膜厚度、隐窝深度、绒毛高度和绒毛表面积均明显大于常规组(P<O.001);超微结构观察发现:常规组移植小肠的粘膜上皮吸收细胞微绒毛短而稀,不规则;多数细胞线粒体肿胀明显,内嵴有断裂现象:上皮基板不完整;而谷氨酰胺组肠粘膜上皮吸收细胞的微绒毛高而密,排列整齐,仅少数细胞可见有线粒体轻度肿胀.组织化学显示:谷氨酰胺组移植小肠粘膜上皮的碱性磷酸酶的阳性面积和积分光密度均显著大于常规组(P<0.001).研究表明:附加3%丙氨酰-谷氨酰胺二肽的全肠外营养可较好的维持移植小肠粘膜上皮吸收细胞超微结构的正常形态和结构的完整性,防止肠粘膜萎缩,改善其吸收功能.Eighteen Wistar rats which received heterotopic small bowel transplantation and total pa-renteral nutrition were randomly divided into two groups and given respectively total parenteral nutrition with 3% alanyl-glutamine (Gln group, n=10) or the standard total parenteral nutrition (STD) group, n = 8) with isonitrogenous balanced nonessential amino acid for ten days. A comparative study on the morphology of the transplanted intestinal sample between the two groups was done by using light microscopy, electron microscopy and histochemical method. The results were as follows : The mucosal thickness, crypt depth, villia height and surface area of the transplanted intestine were significantly greater (P<0.01) in the Gln group as compared with those in the STD group. Ultrastructural observation showed that microvilli of epithelial absorptive cells in the STD group became irregular, shorter and sparser, basal lamina integrity was broken, most mitochnodria were swollen, and chondrial crista were collapsed; but those in the Gln group wereregular, higher and denser, only few cells were observed with swollen mitochondria. Histochemi-cal result showed the positive area and integral light density of AKPase in the epithelium of the intestinal mucous were significantly greater (P<0. 001) in the Gln group as compared with those in the STD group. The results indicated that 3% alanyl-glutaminc enriched total parenteral nutrition could maintain the normal ultrastructure of intestinal epithelial cell, reduce mucosal atrophy and improve its function.
