矽肺与其并发症相关关系的临床分析

摘　　要：[目的]分析矽肺期别与其并发症的相关关系。[方法]收集2004～2006年经新疆乌鲁木齐铁路局职业病诊断组确诊的矽肺患者206例,平均年龄73岁,平均接尘史7.14年。其中Ⅰ期矽肺150例,Ⅱ期矽肺46例,Ⅲ期矽肺10例,分别用χ2检验分析Ⅰ、Ⅱ、Ⅲ期矽肺与慢性阻塞性肺气肿,慢性肺源性心脏病、高血压病、糖尿病、冠心病、呼吸衰竭之间的相关关系。[结果]矽肺期别与慢性阻塞肺气肿及慢性肺源性心脏病之间相关关系分析,χ2值分别为12.28、28.41,均为P﹤0.05,均有统计学差异;矽肺期别与高血压病、冠心病、糖尿病的相关关系分析χ2值分别为0.2027、1.0004、1.4579,均为P﹥0.05,均无统计学差异。[结论]随着矽肺期别的升高,并发慢性阻塞性肺气肿、慢性肺源性心脏病的阳性率逐渐升高;而矽肺期别与高血压病、冠心病、糖尿病、呼吸衰竭的发病率无关。[Objective] To analyse the correlation between the grades of silicosis and its complifcations. [Methods] A total of 206 silicosis patients diagnosed by the occupational disease diagnosis group of Urumqi railway bureau were selected, whose average age was 73 years old, and the mean history of dust exposure was 7.14 years. The Ⅰ grade silicosis was 150 cases. The Ⅱ grade silicosis was 46 cases. The Ⅲ grade silicosis was 10 cases. The method of chi-square test was used to analyse the correlations between the grades of silicosis and chronic obstructive pulmonary emphysema （COPE）, chronic pulmonary heart disease （CPHD）, coronary atherosclerotic heart disease （CAHD）, diabetes mellitus （DM）, hypertention （HP）, respiratory failure （RF）. [Results] As far as the correlations between the grades of silicosis and COPE and CPHD were concerned, the statistic were 12.28, 28.41 respectively （P〈 0.05）. They had statistical significance. Otherwise, there were no significant correlations between grades of silicosis and CAHD, DM, HP, RF, because the x^2 statistic were 0.202 7, 1.000 4, 1.457 9 and 7.943 1 respectively （P〉 0.05）. [Conclusions] With the increase of the grade of silicosis, the positive rates of silicosis combined with COPE or CPHD increase gradually. Whereas the grade of silicosis has no association with the incidence of CAHD, DM, HP and RF.
