欣母沛在妊娠合并特发性血小板减少性紫癜剖宫产时应用价值探讨

摘　　要：目的探讨欣母沛在妊娠合并特发性血小板减少性紫癜剖宫产时的应用价值。方法58例妊娠合并特发性血小板减少性紫癜需手术终止妊娠的孕妇随机分成两组，治疗组30例，剖宫产胎儿娩出后给予宫体注射欣母沛0．25mg及静脉滴注缩宫素20U，宫缩仍不佳再继续应用欣母沛0．25mg；对照组28例，剖宫产胎儿娩山后给予宫体注射缩宫素20U及静脉滴注缩宫素20U，宫缩仍不佳再继续应用注缩宫素20—50U。比较两组术后2小时，24小时的出血量及产后出血的发生率。结果治疗组术后2小时，24小时的出血量明显低于对照组（P〈0.01），治疗组产后出血的发生率明显低于对照组（P〈0.01）。结论妊娠合并特发性血小板减少性紫癜剖宫产时，应用缩宫素和欣母沛比单纯应用缩宫素有减少出血的疗效，并具有方便、快速、安全的特点。Objective To assess application and effects of hemabate for patient with idiopathic thrombocytopenic purpura （ITP） in cesarean section. Methods According to either use of hemabate and oxytocin or oxytocin only after cesarean section, 58 patients who had cesarean section indication were assigned randomly to two groups： treatment group （30cases）, and contrast group （28cases） . The patient in two groups injected hemabate and oxytocin into their uterine corpus iminediately after baby taking out respectively. The total bleeding amounts of patient who underwent cesarean section in two groups during operation and within 24 hours after operation were compared. Results The bleeding amounts at 2 hours and 24 hours after delivery in treatment group were both less than those in contrast group. Conclusion Hemabate is convenient, safe, effective in reduction and prevention of cesarean section postoperative hemorrhage of patient with idiopathic thrombocytopenic purpura （ITP） .
