骨髓增生异常综合征幼稚前体细胞异常定位与PCNA表达的研究

摘　　要：目的：探讨ＭＤＳ骨髓细胞ＰＣＮＡ表达率的变化，ＡＬＩＰ与ＰＣＮＡ表达的关系，为临床上寻找ＭＤＳ向白血病转化的早期监测指标。方法：骨髓组织塑料包埋制片，免疫组化ＡＢＣ法。结果：①正常组，ＭＤＳ组，ＡＭＬ组，ＰＣＮＡ表达率分别为１２．３±１．３、２４．６±２．５、３２．５±４．５（Ｐ＜０．０５）存在显著差异。ＭＤＳ各亚型之间ＰＣＮＡ表达率无明显差异。②ＡＬＩＰ与ＰＣＮＡ表达率有相关性。③ＰＣＮＡ表达率及积分值与骨髓内造血组织面积，幼稚粒细胞数均呈正相关。结论：ＭＤＳ骨髓组织中ＡＬＩＰ阳性病人易转向白血病是由于其异常增殖活性增高所致。ＰＣＮＡ表达率及积分值可作为ＭＤＳ向白血病转化的一种早期动力学指标。Objective:In order to seek for early index for the transformation of MDS to leukemia.To study changing expression rate in bone marrow cell PCNA the relation of PCNA and ALIP.Methods: The plastic embedding smears of myeloid tissue and the immunohistochemical ABC method. Results:The PCNA expression rate(%)for the normal group the MDS group and the AML group was respectively 12.3± 1.3,24.6±2.5 and 32.5±4.5,and significant differences existed among them The PCNA positive expression rate(%)for RA、RAEB and RAEB T of all the MDS subgroups revesled no significant differences;PCNA correlated with ALIP.PCNA expression rate and integral value had positive correlation with the area of the intramedullary hematopoietic tissue and the immature granulocytic number.Conclusion:①The phenomenon that patients with positive ALIP in MDS myeloid tissue had liable conversion to leukemia was caused by the enhancement of the in abnormal proliferating activity; ②The transformation of MDS to leukemia would be surely accompanied by the variation of abnormal proliferating activity.So the PCNA expression rate and integral value and be regarded as a kinetic index for the transformation of MDS to leukemia.
