流行性乙型脑炎患者血清抗体对乙脑病毒蛋白的反应性及其应用

摘　　要：目的：了解流行性乙型脑炎病毒各蛋白成分在刺激机体免疫应答方面所起的作用，并在此基础上建立一种新的而且可供基层医疗卫生单位使用的乙脑快速诊断方法。方法：建立乙脑病毒免疫转印方法，应用免疫转印方法分析乙脑患者血清ＩｇＧ、ＩｇＭ抗体对乙脑病毒蛋白的反应性，并比较免疫转印法与间接免疫荧光法检测乙脑ＩｇＭ抗体差异。结果：应用免疫转印方法检查８９例乙脑病人血清ＩｇＧ、ＩｇＭ抗体对乙脑病毒蛋白的反应情况，发现绝大部分病人血清ＩｇＧ抗体可识别Ｅ蛋白，部分病人尚可识别ＮＳ５，ＮＳ３，ＮＳ１蛋白。说明Ｅ蛋白是乙脑病人血清识别的主要蛋白，在免疫保护方面起着主要作用。从正常人群中筛选出了４５例乙脑隐性感染者，分析隐性感染者血清ＩｇＧ抗体对乙脑病毒蛋白的反应性发现隐性感染者对ＮＳ５蛋白的反应机率明显低于显性感染者。而病人血清ＩｇＭ抗体主要识别Ｅ，ＮＳ５蛋白。免疫转印法检测乙脑ＩｇＭ抗体与间接免疫荧光法比较差异无显著性（χ２＝０５，Ｐ＞０．５），二者的阳性符合率为９５％，阴性符合率为７８．７８％，总符合率为９３．２５％。结论：乙脑病毒Ｅ蛋白是乙脑病人血清识别的主要蛋白，在免疫保护方面起着主要作用。运用免疫转印法检测乙脑ＩｇＭ抗体可早?bjective: In this paper,we want to study the role of Japanese encephalitis virus(JEV)proteins in stimulating immune response of human body,and proceed to develop a new rapid JE diagnosis method which could be used by basic medcine units.Methods: Western blot technique for JE virus was developed and used to detect the serum IgG、 IgM antibodies of JE patients.The difference between the western immunoblot assay and the indirect immunofluorescence assay in detecting the serum JE virus IgM antibody was evaluated.Results: We found that the serum IgG antibodies of the most JE patients recognized the E protein,yet some patients still reacted with NS 5,NS 3,NS 1 protein.The recognization rate of the serum IgG antibodies of JEV inapparent infectioners to JEV NS 5 protein was much less than that of JE patients.The serum IgM antibodies of JE patients mainly reacted wigh E,NS 5 protein.There is no apparent difference between the western immunoblot assay and the indirect immunofluorescence assay in detecting the serum JEV IgM antibody.Conclusion: The E protein was the major protein recognized by JE patients ,it played a major role in stimulating immune response.The western immunoblot assay is reliable for the early diagnosis of JE patients.
