摘  要:目的观察2型糖尿病患者75 g 葡萄糖耐量试验(OGTT)和标准餐试验对胰岛素分泌功能和游离脂肪酸变化的影响。方法 76例病情稳定的2型糖尿病患者接受75 g OGTF 试验,1周后再接受标准餐试验,观察空腹及口服75 g 葡萄糖或标准餐后30、60、120、和180 min 时血糖、胰岛素、C 肽、游离脂肪酸(FFA)和血脂(胆固醇、甘油三酯、高密度脂蛋白和低密度脂蛋白胆固醇)水平。结果 OGTF 与标准餐试验餐后血糖,餐后30 min 为(15.3±3.5)与(9.9±3.4)mmol/L,60 min 为(18.2±4.8)与(12.8±4.0)mmol/L,120 min为(16.3±5.8)与(12.2±4.9)mmol/L,180 min 为(10.6±5.4)与(9.5±4.5)mmol/L,两者比较,差异有统计学意义(F=28.1,P<0.01);胰岛素水平,餐后30 min 为(29±27)与(19±17)μU/L,60 min 为(41±31)与(31±19)μU/L,120 min 为(38±34)与(42±37)μU/L,180 min 为(16±12)与(23±23)vU/L,两者比较,差异有统计学意义(F=8.1,P<0.01)。而 C 肽水平的变化差异无统计学意义。血清 FFA 水平变化标准餐试验高于OGTF,差异有统计学意义(F=4.0,P<O.05);多凶素回归分析显示 FFA 水平与胰岛素敏感性之间的关联具有临界统计学意义(P=0.06),偏相关系数为0.607;而与受试者因素、甘油三酯、胆固醇、高密度脂蛋白胆固醇和低密度脂蛋白胆固醇之间无关联。结论选择标准餐作为刺激物评价2型糖尿病患者胰岛素分泌功能,可减轻患者胰岛的负担,且能够改善胰岛素的敏感性,减少对血 FFA 水平的影响。Objective To observe the effects of 75 gram glucose oral tolerance test (75 g OGTT) and standard mixed meal test (SMMT) on insulin secretion function of the islets of Langerhans and plasma free fatty acid (FFA) in patients with type 2 diabetes mellitus. Methods Seventy-six patients with type 2 diabetes without using insulin and with no obvious complications were recruited for 75 g OGTT following overnight fasting on the first day and SMMT (bread 50 g, egg 50 g and milk 250 ml) on the 7th day. Blood specimens were collected from each patients before the tests and 30 min, 60 min, 120 min and 180 min after glucose or meal load to measure their levels of plasma glucose, serum insulin, C peptide, FFA and lipids (total cholesterol, triglyceride, high-density and low-density lipoprotein cholesterol ). Results No difference in fasting plasma glucose, serum insulin, C peptide, FFA and lipids between 75 g OGTT and SMMT was found. Postprandial plasma glucose 30 min, 60 min, 120 min and 180 min after 75 g OGTT was significantly higher than that after SMMT, with ( 15.3 ± 3.5 ) vs (9. 9 ± 3.4) mmol/L, ( 18. 2 ± 4. 8 ) vs (12.8±4.0) mmol/L, (16.3±5.8) vs (12.2±4.9) mmol/L and (10.6±5.4) vs (9.5±4.5) mmol/L (F =28.1, P 〈0. 01 ), respectively, and serum insulin 30 min, 60 min, 120 min and 180 min after 75 g OGTr was significantly higher than that after SMMT, with (29 ± 27 ) vs (19±17 ) μU/L, (41 ± 31) vs (31±19) μU/L, (38±34) vs (42±37)μU/L and (16±12) vs (23±23) μU/L (F=8. 1, P 〈 0. 01 ) , respectively. No difference in serum C peptide after glucose or meal load between 75 g OGTT and SMMT was found. Changes in serum FFA after SMMT were significantly greater than those after 75 g OGTT, with (422 ± 176) vs (444 ± 170) μmol/L, (336 ± 153) vs (266 ± 139) μmol/L, (246 ± 119) vs (148 ± 88) μmol/L,and (255 ± 113) vs ( 169 ± 139) μmol/L, respectively (F = 4. 0, P 〈0. 05).
