摘 要:目的:探讨CT对下颈椎椎弓根螺钉置入位置的评价。材料和方法:分析154枚下颈椎椎弓根内螺钉置入术后的CT表现,包括螺钉在椎弓根内的位置、角度和损伤或穿破椎弓根骨皮质,并根据其穿出骨皮质的距离分为2度,即I°≤2mm,II°>2mm。结果:CT显示34(22.1%)枚螺钉不同程度地损伤椎弓根外侧骨皮质,其中螺纹穿破椎弓根骨皮质距离I°30(19.5%)枚,II°4(2.6%)枚。结论:CT是评价下颈椎椎弓根内螺钉置入位置的有用方法。CT显示下颈椎椎弓根内螺钉置入术后对椎弓根骨皮质,尤其是外侧骨皮质损伤率较高,但I°损伤,仍属螺钉置入成功和安全。Purpose: To evaluate the postoperative transpedicle screw placement of the lower cervical pedicle by CT. Materials and Methods: The CT features of 154 postoperative transpedicle screw placement were analyzed retrospectively, including the screw position, angle,cortical injury, and according to the distance of the screw thread perforated pedical cortical bone the cortical injuries were divided into two degree, namely Ⅰ° 〈 2mm and Ⅱ° 〉2mm. Results: Of 154 postoperative transpedicle srew placement, CT showed that the pedicle cortical bones were often injured by the screw thread. There were 34(22.1% ) screws perforated the cervical pedicle with Ⅰ° and 30( 19.5% ) screws , with Ⅱ°. Conclusions: CT is an useful method in assessing postoperative transpediclc screw placement of low cervical pedicle. On CT the pedicle cortical bones are usually injured by the screw threads, whereas if the cortical injury with Ⅰ°, the pedical srew placement is still successful and safe.
