补肾益气法对流产大鼠胎盘形态学的影响

摘　　要：目的了解流产模型及补肾益气方治疗自然流产后大鼠的胎盘病理学改变,在组织形态学上探讨补肾益气法对流产的治疗作用,寻找其作用的形态学基础。方法将SD大鼠分为正常对照组、溴隐亭模型组(孕6～8d皮下注射溴隐亭0．45mg·kg^(-1)·d^(-1))、中药低剂量组(孕1～11d灌服中药1．4g·kg^(-1)·d^(-1),同时孕6～8d皮下注射溴隐亭)、中药高剂量组(孕1～11d灌服中药2．8 g·kg^(-1)·d^(-1),余同中药低剂量组)和黄体酮组(在溴隐亭模型的基础上,孕6～11d肌内注射黄体酮8mg·kg^(-1)·d^(-1))。应用光学和电子显微镜观察大鼠胎盘。结果①光学显微镜下表现:中药低、高剂量组的绒毛血窦表面积逐渐增大,血管含量丰富,管腔形态规则,着床部位蜕膜腺体丰富;同时还观察到血管合体细胞膜明显增多。②电子显微镜下表现:与正常对照组相比,在合体滋养层,溴隐亭模型组的胎盘组织游离面的血窦密度明显降低,数量较多的线粒体肿胀,粗面内质网池扩张,膜上核糖体有脱颗粒现象,基底膜弯曲、增厚;在基底膜内或附近常见电子致密球状颗粒;微血管内皮增厚,伴有微绒毛样突起。中药低、高剂量组和黄体酮组滑面内质网增加,呈扩张状态,线粒体丰富,胞质内有大量脂滴,其他细胞器基本正常。结论溴隐亭引起大鼠胎盘结构改变,影响其进行物质交换和分泌激素的功能,从而导致流产。补肾益气中药作用后可逆转流产胎盘结构的形态学改变,使其向有利于甾体激素合成的方向转化,从而达到维持妊娠的目的。Objective To observe the effect of bushen yiqi medicinals on the rat placental morphology of rats after natural abortion, so as to search for methods for maintenance of pregnancy. Methods The Sprague-Dawley rats were divided into Bromocriptine group and control group. Rats in Bromocriptine group were further divided into low and high dose Chinese medicinals groups and progesterone group. Light and electron microscope were used to observe the morphological changes of rats placenta. Results The surface area of placental villi blood sinusoids was enlarged, blood vessel were abundant and lumen regular under light microcope. Under the electron microscope, the basement membrane of the trophoblast epithelium was thickened and tortuous in the placental tissue of model group. There were large number of dense microspherical particles, with mitochondrial swelling. The cisternae of the rough endoplasmic reticulum（rER） were dilated and contained flocky material; degranulation of ribosomes on the rER were also noticed. The capillary endothelium was thickened with microvilli-like projections. Chinese medicinal groups and progesterone group both showed increased smooth endoplasmic reticulum（sER）, there were plenty of mitocbondria and lipid droplets in the cytoplasm; the other organelle were basically normal. Condmion 13romocriptine can induce changes in placental structure and influence normal placental function. Bushen yiqi medicinal therapy can reverse the morphologic changes of placenta structure after abortion improve the synthesis of female hormones, eventually contribute to the maintenance of pregnancy. （Shanghai Med J, 2007, 30：453-455）
