血清骨桥蛋白和CA_(125)联合检测在卵巢上皮癌诊疗中的应用价值

摘　　要：目的探讨血清骨桥蛋白(osteopontin OPN)和CA125在卵巢上皮癌诊疗中的应用价值。方法对卵巢上皮癌患者64例、卵巢良性肿瘤患者68例治疗前血清样品及48例健康妇女血清样品,分别用双抗体夹心酶联免疫吸附法(ELISA)及微粒子化学发光法测定其OPN和CA125水平,并对卵巢上皮癌64例患者术后3个月内连续测定,观察疗效和OPN、CA125变化间的相关性。结果卵巢上皮癌患者血清OPN、CA125水平明显高于卵巢良性肿瘤组及健康妇女组。OPN检测敏感度为82.8%;CA125检测敏感度为71.9%;联合检测敏感度93.8%。原发性卵巢上皮癌64例患者中,疗效为完全缓解(CR)的19例术后OPN、CA125水平较术前明显下降(P<0.01);疗效为部分缓解(PR)的30例术后OPN、CA125值较术前下降(P<0.05);疗效为进展(PD)的10例术后OPN、CA125值较术前升高(P<0.05);疗效为无改变(SD)的5例手术前后OPN、CA125值差异无显著性(P>0.05)。结论OPN测定是卵巢上皮癌诊断和监测疗效的有效检查方法,联合检测CA125可提高其临床应用价值。Objective To investigate the application value of combined detection of serum levels of OPN and CA125 in the diagnosis and treatment of ovarian epithelial cancer. Methods Serum levels of OPN and CA125 in 64 patients with epithelial ovarian cancer,68 patients with benign ovarian tumor and 48 healthy subjects were measured by ELISA and microparticle chemiluminescence method before treatment. The successive detectio nof the levels of OPN and CA125 in 64 patients with epithelial ovarian cancer for 3 months was carried out to observe the relationship between the therapeutic effect and the changes of the levels of OPN and CA125. Results The levels of OPN and CA125 in patients with ovarian epithelial cancer were significantly higher than those of benign ovarian tumor group and control group. The sensitivity and the specificity of OPN and CA125 were 82.8%, 91.7% and 71.9%, 89.6% respectively. The sensitivity of combined detection of two markers was 93.8%. Serum OPN and CA125 decreased signiticantly after operation in 19 patients with complete remittence（ P 〈 0.01 ）, in 30 patients with partial remittence （ P 〈 0.05）, which increased significantly in 10 patients with progress of disease （ P 〈 0.05 ） and 5 patients had no change （ P 〉 0.05 ）. Conclusion The measurement of serum OPN is an effective method for the diagnosis and monitoring of the therapeutic effect of ovarian epithelial cancer. The clinical value can be improved by combined detetcion of OPN and CA125.
