One stage urethroplasty for hypospadias by the use of midline scrotal flap

摘　　要：总结１９８６年７月～１９９５年１０月行一期正位尿道下裂修复术３４２例，手术成功率９５．６％。其中阴茎型１５６例，阴茎阴囊型１０２例，阴囊型６８例，会阴型１６例。采用阴囊纵隔血管蒂皮瓣成形尿道加阴茎头腹侧高位Ｖ形切口，将成形尿道口做到阴茎头顶端，对１８４例病人采用保留导尿替代传统的尿液转流。本法最适宜治疗阴茎型和阴茎阴囊型尿道下裂。由于皮瓣血供良好，皮管翻转固定后，其缝合面紧贴阴茎海绵体腹侧，极少发生尿瘘及尿道狭窄等术后常见并发症。One stage urethroplasty with the use of midline scrotal flap was carried out for 342 cases of hypospadias,156 of them being of the penile type,102 the penoscrotal junction type,68 the scrotal type and 16 the perineum type.Midline scrotal raphe flap with vascula rized pedicle was used for urethral reconstruction and a high V shaped incision was made on the ventral surface of the glans so as to put the neomeatus to the tip of the glans.Maintained catheterization via the neourethra for urine drainage instead of the conventional urinary diversion was instituted in 184 cases.The overall success rate was 95.6% (327/342).This technique was best indicated for hypospadias of penile and peno scrotal junction types.As the scrotal raphe flap was well vascula rized and the rotated neourethra suture line butted well against the ventral surface of the cavernosa complications such as urethral fistuli and urethral stricture seldom occurred.
