摘  要:目的比较3种雾化吸入方法对慢性阻塞性肺疾病(COPD)患者动脉血气分析及临床症状的影响。方法将60例COPD病人随机分为3组,每组20例,用相同剂量的安溴索和特布他林雾化混悬液,分别采用超声雾化(A组)、空气压缩雾化(B组)、氧驱雾化(C组),雾化吸入前后分别采集动脉血做血气分析,同时观察病人临床症状的变化。结果3组雾化吸入前后比较,pH及PaCO2差异无显著性意义(P>0.05),PaO2及SaO2A组降低(P<0.05),而C组升高(P<0.05)。临床症状加重的病人数A组多于B组(P<0.05)、C组(P<0.01)。结论氧驱雾化和空气压缩雾化均优于超声雾化。而氧驱雾化又优于空气压缩雾化,可作为COPD病人优选的雾化吸入方法。Objective To observe the effect of three different nebulization inhalant methods for patients with chronic obstructive pulmonary disease(COPD) in arterial blood gas analysis and symptom. Method 60 patients with COPD were randomly divided into three groups: group A( used ultrasound nebulization ), group 13 (used compressed air nebulization) and group C(used oxygen nebulization), each group with 20 cases. All the patients were treated with the same dose of ambroxol and terbutaline. Arterial blood gas was measured and clinical symptoms were assessed before and after treatment respectively. Result Before and after nebulization inhalation, there were no significant differences in pH and PaCO2 (P〉0. 05) among three groups; however Group A had significant decreases in PaO2 and/SaO2 (P〈0.05), while group C had significant increases (P〈0.05). The number of patients/whos symptoms were getting worse in group A was more than that in group B(P〈0.05), as well as much more than group C(P〈0.01), Conclusion Oxygen nebulization inhalation and compressed air nebulization inhalation are better than ultrasound nebulization inhalation, and oxygen nebylization inhalation is a safety method, in treating the patients with COPD.
