摘　　要：目的:评价A型肉毒毒素治疗咬肌肥大的远期疗效。方法:对18例(34侧)咬肌肥大患者进行A型肉毒毒素肌肉注射治疗。其中16例双侧咬肌肥大就医者每侧注射剂量为50U/1ml,2例单侧咬肌肥就医者每侧注射剂量为60U/1ml。观察治疗前后不同时期就医者咬肌厚度变化情况及不良反应和并发症发生情况。结果:治疗后不同时期的咬肌厚度均较治疗前减小(P<0.001),药物注射后4～12周咬肌厚度减小最明显。平均随访时间78.5±18.8个月,患者总体满意率为83%,无严重并发症发生。结论:A型肉毒毒素可以替代手术治疗咬肌肥大。Objective To study the safety and efficacy of botulinum toxin A in the treatment of masseteric muscle hypertrophy. Methods Eighteen patients （34 sides） were treated by intramuscular injection of botulinum toxin A： 50U/lml for each side of 16 patients with bilateral masseteric hypertrophy; 60U/lml for 2 patients with unilateral hypertrophy. The masseter thickness measured by ultrasonography and complications were recorded at different time. Results The masseter thickness at different time post injection was reduced significantly （P〈0.001）. Maximum reduction was recorded at 4-12 weeks after injection. The mean follow-up period was 78.5±18.8 months. Eighty four percent of the 18 patients were generally satisfied. No long-term major complications appeared. Conclusion Botulinum Toxin A injection is a safe and effective long-term treatment for masseteric muscle hypertrophy, which can replace the masseter resection.
