宫颈内口功能检查在预防早产、流产中的作用

摘　　要：目的:探讨宫颈内口功能检查在预防早产和流产中的作用。方法:对160例不孕和有流产史经治疗后妊娠的病例进行孕早、中、晚3期宫颈机能检查,采用自行设计的在阴道内置水囊后进行腹部超声检查,测量宫颈的各径线,如果符合宫颈机能不全的诊断标准,则行宫颈环扎术。结果:宫颈机能不全的发生率为13.75%,且随流产次数的增加而升高。宫颈机能不全孕妇经过宫颈缝扎术后的早产发生率为8.30%,显著低于未行宫颈缝扎术的早产率(80%),P<0.05。且宫颈机能不全而未行缝扎术者,虽给予镇静、保胎等治疗,仍有20%发生流产,而宫颈机能正常或不全但行缝扎术后,无流产发生,P<0.05。结论:对于宫颈机能不全的早期诊断、早期治疗可显著降低患者的早产和流产的发生率,值得临床推广。Objective： To explore the important role of detecting the function of enternal cervial os for preventing premature labor or spontaneous abortion. Methods： A total of 160 pregnant women with previous spontaneous abortion and infertility were received treatment in our department until they were all pregnant, and received detection of the function of enternal cervical os in first, second and third trimester of pregnancy, respectively by a method designed by ourself. Transabdominal ultrasonographic scan was used under the station of placement a water pocket into vaginal to measured the cervical diameter. The diagnosis of dysfunction of entemal cervical os was established when any one of the cervical diameter was beyond critical values. Results： The rate of dysfunctional enternal cervical os was 13.75% in pregnant women. The rate of dysfunctional enternal cervical os was higher with the number of spontaneous abortion increased. In pregnant women of dysfunctional enternal cervical os, the rate of premature labor and late abortion was 8.30% and 0 after the suture of cervical os, then it was 80.00% and 20.00% without the suture of cervical os, respectively （P〈0.05）. Conclusion： Such a diagnostic criteria and unique detecting method have a larger use value for diagnosis and treatment of the dysfunction of entemal cervical os early, therefore, it is worth recommendation.
