摘 要:目的:探讨特发性身材矮小(idiopathic short stature,ISS)儿童血清胰岛素生长因子-1(insulin-like growthfactor-I,IGF-1)水平及其临床意义。方法:36例ISS儿童及正常对照组儿童50例,分为青春期前组及青春期组。测定其身高、体质量,体质量指数形态指标,采用CHN法评定左侧手、腕部骨化指标,计算骨龄。采用ELISA法测定其血清IGF-1。结果:①ISS组和对照组血清IGF-1水平与年龄均存在较强的正相关关系(r=0.425,r=0.716,P<0.01)。IGF-1随年龄的增长而升高,青春期组血清IGF-1水平高于青春前期组;②青春前期及青春期ISS儿童血清IGF-1水平分别为154.33±56.53ng/ml和223.08±75.70 ng/ml,正常组为237.28±72.12 ng/ml和371.99±43.99 ng/ml,ISS组血清IGF-1水平低于正常对照组,存在显著性差异(P<0.01)。结论:ISS儿童存在生长激素-IGF-1轴功能异常,IGF-1是骨骼生长原始动力,可用以判断儿童骨骼生长状况。Objective: To measure concentrations of serum insulin -like growth factor (IGF) - Ⅰ and try to explore the levels and clinical significance of IGF - Ⅰ in children with idiopathic short stature (ISS) . Methods: 36 ISS and 50 normal children were divided into two groups to measure the height, weight and body mass index. Left hand and wrist were measured with CHN way for bone age. Serum IGF - Ⅰ was measured by enzyme linked immunosorbent assay (ELISA) . Results: ①There was significant positive correlation between age and the serum level of IGF - Ⅰ. The concentrations of IGF - Ⅰ was gradually increased with aging and higher in adolescence group than that in preadolescence group. ②IGF - Ⅰ in ISS group of preadolescence and adolescence showed ( 154. 33 ± 56. 53) and (223. 08 ± 75. 70) ng/ml, but (237. 28 ±72. 12) and (371. 99 ±43. 99) ng/ml in normal children. There was significant difference between ISS and control in both age in the levels of IGF - Ⅰ (P 〈0. 01 ) . Conclusion: There is disorder in growth hormone - IGF - I axis of the children with ISS. IGF - Ⅰ is original power to bone growth and could be used to estimate the state of bone growth with ISS.
