摘　　要：目的探讨关节镜下自体髌腱中1/3重建前交叉韧带(ACL)术效果的影响因素。方法对15例前交叉韧带损伤患者,在关节镜下行自体骨-髌腱-骨(B-PT-B)中1/3重建ACL。其中同时行侧副韧带修复1例,半月板损伤部分切除3例,半月板结合部缝合1例。结果按照日本骨科学会制定的膝关节疗效评定标准,优6例,良7例,中2例,优良率为86.7%。结论术中选择等长点,测量关节内重建韧带的长度,获取标准的髌腱两端骨块及胫骨、股骨隧道,并正确使用挤压螺钉固定两骨块于隧道中是减少并发症、提高手术效果的关键。该手术创伤小,对关节内环境影响小,可同时进行关节内其他手术,术后恢复快。Objective To discuss the clinical outcome of anterior cruciate ligament （ACL） ＇ s reconstruction with autologous bonepatellar-tendon-bone （B-FF-B） transplantation under arthroscope and its influencing factors. Methods ACL＇s reconstruction with B-PT-B strip （ middle 1/3 ） was conducted on 15 patients With anterior cruciate ligament＇ s injury, among whom one was performed with collateral ligament＇ s repair, two with ablation of injured meniscuses, and one with suture of meniscal juncture. Results According to the criteria of knee joint function evaluation set up by Japanese Association of Orthopedics, the therapeutic outcome was excellent in six, good in seven, and moderate in two patients. The rate of excellent and good outcome was 86.7%. Conclusion ACL＇ s reconstruction with eligible B-PT-B acts as a good therapeutic way to anterior cruciate ligament＇ s injury and is worthy to be spread widely. Not only it has the virtue of minimal damage to tissue, but also the other disorders within the knee joint can be dealt with at the same time.
