平片式与疝环充填式无张力修补术的临床应用比较

摘　　要：目的比较平片式无张力疝修补术与疝环充填式无张力修补术治疗腹股沟疝的临床应用价值。方法选择腹股沟疝患者159例,随机分为2组,I组89例/114侧,采用平片式无张力修补术,II组70例/93侧,行疝环充填式无张力修补术。比较两组的手术时间、住院时间、术后疼痛持续时间、恢复工作时间、住院总费用、并发症发生率和复发率。结果1组患者与2组比较在手术时间、术后恢复工作时间、术后疼痛、术后复发率和并发症方面差异无统计学意义(P>0.05),但在住院总费用上比较1组明显少于2组,差异有统计学意义(P<0.01)。结论平片式无张力疝修补术和疝环充填式无张力修补术治疗腹股沟疝疗效肯定,但平片式无张力疝修补法费用低廉,更值得在临床上推广应用。Objective To compare the therapeutic results of Lichtenstein＇s open tension - free bernioplasty and plug mesh hernia repair. Methods 159 patients with inguinal hernia were randomly divided into 2 groups： Lichtenstein＇s open tension - free hernioplasty was performed in 89 patients/114 sides （ group 1 ）, and plug mesh hernia repair was performed in 70 patients / 93 sides （ group 2）. Operation time, hospital stay, duration of postoperative pain, time to work, cost, complications and recurrent rate were compared between the two groups. Results There were no significant differences in operation time, time to work, duration of postoperative pain, complications and recurrent rate between the two groups（ P 〉 0.05）, but the cost was significantly higher in group 2 than in group 2. Conclusion Lichtenstein＇s open tension - free hernioplasty and plug mesh hernia repair are effective techniques for inguinal hernia, but Lichtenstein＇s hernioplasty costs less.
