超声显像在浅表淋巴结肿大的良恶性鉴别中的价值

摘　　要：目的：通过淋巴结的超声显像对淋巴结肿大的良恶性进行鉴别与预测。方法：使用高频率超声探头对颈部、腋窝及腹股沟浅表淋巴结探查，观察淋巴结大小、形态及回声改变。结果：恶性淋巴结的横径及横径／纵经比值增大。其与良性淋巴结肿大有非常显著性差别（P〈0．01）。以横径〉0．58cm为参考值，恶性淋巴结预测的敏感性100％、特异性58％、准确性64％，而以横径／纵径〉0．46为参考值，其敏感性98％、特异性62％、准确性75％。结论：淋巴结超声显像对淋巴结肿大的良恶性鉴别有较高的准确性，它为穿刺活检初步筛选病员，对肿瘤性疾病的早期诊断、分期及治疗有非常重要的意义。Objective By imaging superficial lymph nodes for distinguishing the characteristics. Methods Checking the axillary, cervical and inguinal and inguinal lymph nodes by high frequce probe and observing the change of shapes, Diameter and echo. Results The bread diameter and ratio of breadth and length become length become larger in malignant nodes. There is significant difference in malignant and benign. Using bread 〉0. 58 cm as a parameter, the sencifivity（ 100% ） specificity （58%） and the accuraty（64% ） can be acquired, using ratio 〉 0.46 ,the sencitivity（98% ）specificity （60%） and the accuraty（75% ） can be acquired. Conclusion It is accurated than lymph nodes could be distinguished by ultrasound and it is valuable to select and diagnose and treat patients early to select and diagnose and treat patients early in tumour.
