人感染猪链球菌病脑膜炎型临床特点分析

摘　　要：目的:分析四川省成都地区7～8月发生的人感染猪链球菌病脑膜炎型临床特点,为更好地控制、治疗该病提供依据。方法:对20例人感染猪链球菌病脑膜炎型进行回顾性分析。结果:20例人感染猪链球菌病脑膜炎型均为男性,平均年龄32～76岁,有皮肤伤口及与病死猪接触史。起病急,临床表现为畏寒、寒战、发热及头痛、恶心、呕吐及脑膜刺激征、病理征,部分病人伴听力下降和口唇疱疹。脑脊液呈化脓性脑膜炎改变,血液、脑脊液中分离出7株(35%)猪链球菌2型。头孢曲松钠联合青霉素治疗有效,全部治愈。结论:本次人感染猪链球菌病脑膜炎型由猪链球菌Ⅱ型所致,病死猪为传染原;脑膜炎型比例高,对头孢曲松钠联合青霉素治疗有效,早期治疗预后较其他亚型好。Objective： To analyze the clinical characterization of 20 patients with streptococcus suis infection - meningitis type. Methods： The clinical data of 20 patients with streptococcus suls infection - meningitis type were retrospectively reviewed. Results： All the patients were males, age 32 ～ 76 years, and have the history to directly contact disease/dead pig and most have cutaneous lesion. Clinical feature included fever with chill, nausea, vomiting, headache and stiff neck. Some patients have herpes labialis and heating loss. Streptococcus suis growth from blood and cerebrospinal fluid culture samples was found in 7 cases. CSF analysis revealed purulent. Treatment with ceftriaxone and Penicilin can be very effective and all the patients recovered. Conchision： S. suis type 2 caused the patients with streptococcus suis infection and the diseased/dead pig is the source of infection. The ratio of S. suis meningitis type is high, and it is sensitivity to ceftriaxone and Penicilin. The prognosis of S. suis meningitis type was better than other serotype.
