摘　　要：目的探讨心力衰竭(HF)患儿血清基质金属蛋白酶-2(MMP-2)水平变化及临床意义。方法测定HF组和健康对照组的血清MMP-2和血浆N末端脑利钠肽(NT-proBNP)水平,同时分析HF组血清MMP-2与血浆NT-proBNP之间相关性。结果HF组血清MMP-2水平显著高于健康对照组,且随心衰程度的加重而显著增加(P<0.01);MMP-2与NT-proBNP呈正相关(r=0.78P<0.01)。随访6个月发现,MMP-2>600μg/L且NT-proBNP>500pmol/L的HF患儿病死率和再入院率明显高于仅具二者之一者。结论血清MMP-2参与HF的左室重塑,其血清水平的检测有助于动态评估HF病情严重程度及预后。Objective To investigate the changes and clinical significance of serum matrix metalloproteinase -2 （MMP -2） concentration in children with heart failuro （HF）. Methods Serum MMP - 2, plasma N - terminal pro - brain natriuretie peptide （ NT - proBNP） concentrations were all measured in HF and control group, and the relationship between serum MMP - 2 and NT - proBNP was also studied. Results The serum MMP - 2 concentration in ehildron with HF was significantly higher than that in control group, and it also increased with the increased severity of heart failure （ P 〈 0.01 ）. The serum MMP - 2 concentration and plasrna NT - proBNP had significant correlations （ r = 0.78 P 〈 0.01 ）. Six months following the mortality and rehospitalization of children with MMP - 2 〉 600 μg/L and NT - proBNP 〉 500 pmol/L were greater than those in any group. Conclusion Serum MMP -2 can be involved in left ventricular remodeling of HF,and measuring its concentration is helpful to judge the severity and prognosis of HF.
