摘 要:目的分析解放军第98医院临床分离的4种非发酵糖革兰阴性杆菌中氨基糖苷类修饰酶(AM E s)基因存在状况。方法在2000年7月至2004年10月间从临床分离60株鲍曼不动杆菌(A b)、30株铜绿假单胞菌(Pa)、19株嗜麦芽窄食单胞菌(Sm)和15株黄杆菌属细菌(Fb),采用PCR及序列分析法分析9种AM E s基因aac(3)-Ⅰ、aac(3)-Ⅱ、aac(3)-Ⅲ、aac(3)-Ⅳ、aac(6')-Ⅰ、aac(6')-Ⅱ、an t(3'')-Ⅰ、an t(2'')-Ⅰ和aph(3')-Ⅵ。结果A b、Pa、Sm和Fb中9种AM E s基因总阳性率分别为81.7%、86.7%、10.5%和0.0%。A b、Pa和Sm中分别检出8种、5种和2种基因。aac(3)-Ⅲ基因均阴性,各菌种中阳性率最高的基因分别是Pa中aac(3)-Ⅱ(76.7%)、A b中an t(3'')-Ⅰ(60.0%)、Sm中aac(6')-Ⅱ和an t(2'')-Ⅰ(均为10.5%)。结论临床分离的非发酵糖革兰阴性杆菌(除Fb外)中至少存在8种AM E s基因,Pa及A b中AM E s基因携带率高。Objective To investigate the aminoglycoside modifying enzymes (AMEs) genes in 4 kinds of nonfermenting gramnegative bacilli isolated from the 98th Hospital of PLA. Methods 60 strains of Acinetobacter baumannii (Ab), 30 strains of Pseudomonas aeruginosa (Pa), 19 strains of Stenotrophomonas maltophilia (Sm), and 15 strains of Flavobacterium (Fb) were isolated from hospitalized patients between Jul, 2000 and Oct, 2004. 9 kinds of AMEs genes of aac (3) - Ⅰ, aac (3) - Ⅱ , aac (3) -Ⅲ , aac (3) -Ⅳ , aac (6') -Ⅰ , aac (6') -Ⅱ , ant (3'') -Ⅰ , ant (2'') -Ⅰ and aph (3') -gl were analyzed by PCR and verificated by DNA sequencing. Results In Ab, Pa, Sm, and Fb, the total positive rates of 9 kinds of AMEs genes were 81.7%, 86.7%, 10. 5%, and 0.0%, respectively. 8 kinds of AMEs genes were positive in Ab, 5 kinds of in Pa, and 2 kinds of in Sin, respectively. Gene of aac (3) -Ⅲ were not found in these strains tested . It was upmost that the positive rates of genes of aac (3) - Ⅱ and ant (3'') -Ⅰ were 76.7% and 60.0% in Pa and Ab, respectively. Conclusion The harbored rate of AMEs genes is very high in Pa andAb, and at least 8kinds of genes exist includingaac (3) -Ⅰ , aae (3) -Ⅱ , aac (3) -Ⅳ, aac (6') -Ⅰ , aac (6') -Ⅱ , ant (3'') -Ⅰ , ant (2'') -Ⅰ and aph (3') -Ⅵ in nonfermenting gram-negative bacilli isolated except for Fb.
