摘　　要：目的:比较3种HLA-B27检测方法的敏感度、特异性、适用性。方法:采用微量淋巴细胞毒试验、PCR-SSP和流式细胞仪方法同时检测128份待检标本HLA-B27。结果:淋巴细胞毒试验、PCR-SSP和流式细胞仪方法敏感度、特异性、阳性预测价值、阴性预测价值分别为0.468、0.975、0.917、0.759;1.000、0.988、0.979、1.000;0.978、1.000、1.000、0.988。PCR-SSP和流式细胞仪方法敏感度显著性高于淋巴细胞毒试验,两两比较P<0.01;阴性预测价值优于淋巴细胞毒试验,两两比较P<0.05。结论:PCR-SSP和流式细胞仪方法均优于淋巴细胞毒试验,具有较好的临床适用性。Objective ：To evaluate the diagnostic values of three methoods in detecting HLA - B27. Methods： Micro - complement dependent cytotoxicity test, PCR -SSP, flow cytometry were used to detect HLA -B27 in 128 samples. Results：The sensitivity, specificity, predictive value of a positive test, predictive value of a negative test of cytotoxicity test, PCR - SSP, flow cytom- etry were 0. 468,0. 975,0. 917,0. 759 ; 1. 000,0. 988,0. 979,1. 000 ;0. 978,1. 000,1. 000,0. 988 ,respectively. The sensitivities of PCR - SSP and flow cytometry were higher than those of cytotoxicity test, P 〈0. 01. there were obvious differences in the negative predictive values between PCR- SSP and cytotoxieity test, flow eytometry and cytotoxieity test, P 〈 0. 05. Conclusion： The diagnostic values of PCR - SSP and flow cytometry in decting HLA - B27 are higher than those of cytotoxicity test. So it can be used in clinical diagnosis.
